- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested for this solicitation. This solicitation VA250-15-Q-0269 is being issued as a Request for Quotation (RFQ), with the intent to award a Firm-Fixed Price contract for services. All solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-60. [Firm-Fixed-Price] The contractor shall complete the work required under this solicitation as stated in the Statement of Work (SOW), unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor's proposal, the contractor's proposed completion date shall prevail. This requirement is restricted for small business for commercial items under the North Atlantic Industrial Classification System (NAICS) 541990; size standard/$15 million per annum. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, lowest price technically acceptable (LPTA) and received via email to the Point of contact Adoniss Gibson at Adoniss.Gibson@ va.gov. All offers are due by 1:00pm Tuesday, June 09, 2015. The following commercial service is requested: Statement of Work - Annual Medical Gas Inspection Annual Evaluation of the piped OXYGEN, NITROUS OXIDE, MEDICAL AIR, DENTAL AIR, MEDICAL VACUUM, DENTAL VACUUM, NITROGEN and EVACUATION/WAGD Systems, in accordance with JCAHO (Joint Commission on Accreditation of Healthcare Organizations) 2012 standards, CGA (Compressed Gas Association) standards, NFPA 99 (National Fire Protection Association) 2012 standards, ASSE (American Society of Sanitary Engineering 6000, 2006), and USP/NF (United States Pharmacopeia Convention) requirements and recommendations. All equipment is located on the Dayton VAMC Campus located at 4100 West 3rd Street, Dayton OH 45428. All work shall be completed by September 30, 2015 and a written report shall be provided within 30 days after onsite work. The report shall include all findings, note what was checked and the location, and reference applicable code sections. The report shall also provide a concise listing of discrepancies for inlet/outlets, alarm panels, source equipment, and zone valves (example provided below); this concise listing shall provide location (building and floor), room number, discrepancy, NFPA code that discrepancy applies to, recommended resolution for discrepancy, and any additional notes. Deficiencies noted shall be communicated to the COTR within 24 hours of discovery and include a recommendation for remediation. The contractor should provide adequate staff to monitor up to 3 master alarm panels and sources when checking the source alarms (minimum of 4 contractors needed). The contractor may have to work outside the usual hours of 7am-4:30pm due to clinical need in some areas such as surgery, recovery room, GI clinic and emergency department. Work shall be coordinated with the Biomed department. Inspection shall include checking of all source valves. The VA will provide a technician to accompany the contractor when checking source valves. Contractor shall check ALL outlets/inlets, skipping those in use and noting them on the report. All bidders must be certified to inspect medical gas systems and shall include a copy of the certification with their final report.
