THIS IS A SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS The Department of Veterans Affairs is conducting a MARKET RESEARCH SURVEY to obtain information regarding: (1) the availability and capability of qualified small and large business sources; (2) if small business, whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concerns; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. Small business concerns should be capable of furnishing all labor, supervision, and parts, to include all equipment necessary to perform all work require under the proposed contract. The NAICS code is 518210 Place of Performance: Durham VA Medical Center, 508 Fulton Street, Durham, NC 27705 Section A. Industry Survey Responses Requested: Sources Sought which can provide website hosting and data collection services for online survey Scope of Work: Contractor shall construct and host a web site to implement a public internet based survey of caregivers of Veterans. 1) Hosting facility must have FISMA (Federal Information Security Management Act) high certification. 2) A temporary authority to operate (TATO) or authority to operate (ATO) with the Department of Veteran Affairs will be necessary. Authorities to operate and temporary authorities to operate are obtained and managed through the directions outlined in this handbook: http://www1.va.gov/vapubs/viewPublication.asp?Pub_ID=733&FType=2. 3) The Web site must be ready for validation testing no later than 01 Aug 2015, and ready for full operation no later than 01 Sep 2015. 4) Surveys will be requested from approximately 20,000 people. Invitations to survey will be issued over a period of a few days beginning 01 Sep 2015 and equipment scaling must support possible response by majority of invitees over a period of a few days. VA CARES will be responsible for sending out the invitation prior to 01 Sep 2015. 5) Survey will be open for one month. 6) Survey definition currently exists as a word document and in electronic format, in DatStat Illume software. VA CARES will provide the survey to vendor in both electronic and paper forms. The vendor will convert survey to the survey package format used by vendor. Survey definition includes: a.Questions i.Free Text answers ii.Multiple Choice answers iii.Select all that apply iv.Multi-control tables v.Multiple numeric responses that must equal a certain value before progressing with the survey. b.Skip directions (depending upon answers to a given question or questions, in some cases next question will vary) 7) Survey tool must provide for partial completion, and resumption of survey at a later time. The survey is about 100 questions, many of which have multiple parts. The survey is about 20 pages in a word document. The 8) Intermediate results (completed survey data) shall be provided to VA on a weekly basis throughout the one month long period the survey is open per the requirements outlined in Section A. 3.9 (below). 9) Weekly data transfers to the VA will be accomplished by generating an MS-SQL database backup file each week. After generation, each backup file will be encoded into a self-extracting ZIP file encrypted to Federal Information Processing Standards (FIPS)140-2 standards. Personnel at the Durham VA will use CuteFTP or equivalent software to connect to a SFTP (secure file transfer process) server established by the vendor, to accomplish a VA-initiated secure transfer of the file from the vendor server to a VA machine. After fetching a file, VA personnel will decrypt it/unzip it. Vendor must provide name and IP address of SFTP server at least one month prior to date of expected first transfer, to enable registration of the machine on the VA's firewall. 10) Data storage must be kept segregated from other data on vendor systems, including on backup systems. Following final successful transfer of data at end of survey period, all vendor copies, including tape, SAN snapshots or replications, or any other existing backups, must be destroyed, and certified by vendor as having been destroyed. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing this type of service. Please include the following: (a) staff expertise including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. 3) Submit Capability Statements by June 9, 2015 to: Network Contracting Office 6, ATTN: Sarita Foxworth. Responses should be received no later than 8:00 AM EST. Submissions must be made via email to: Sarita.foxworth@va.gov. NO CAPABILITY STATEMENTS MAY BE SUBMITTED AFTER JUNE 9, 2015. AT THIS TIME NO SOLICITATION EXISTS. (DO NOT REQUEST A COPY OF THE SOLICITATION). A FUTURE NOTICE WILL BE POSTED WHEN THE SOLICITATION WILL BE RELEASED. 4) Disclaimer and Important Notes: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. (5) No Proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s.)
