- SCOPE OF WORK/SYNOPSIS (VHA SOP 160-010-01 Item #4) PROJECT No: 679-15-R02 PROJECT TITLE: Boiler Plant Modifications/Upgrades DESCRIPTION OF PROJECT Contractor shall furnish all labor, material, equipment, and supervision to: (1) install two new isolation valves on the line feeding each condensate tank and (2) install separate chemical feed lines to each boiler from VA-provided chemical feed tanks. (1) Currently there is no safe method to isolate each condensate tank without physically climbing over and on top of high-pressure steam lines to isolate the header to each tank. This is unsafe for boiler operator personnel and presents a high risk of injury. Installation of a new valve to each tank, along with a chain link remote operator to open/close it from ground level will eliminate this unsafe work condition. It will allow Boiler Plant personnel to easily isolate each condensate when performing maintenance. Contractor shall perform the following work to install isolation valves: o Provide scaffolding as necessary to access the incoming condensate line. o Cut into existing lines to split each tank and install companion flanges and 3" 150 psi gate valves. o Provide chain link remote operator to open/close each valve from boiler plant floor level. o Re-insulate removed insulation piping. Prior to starting this work, Contractor shall coordinate with boiler plant personnel to ensure that the condensate lines to be worked on are isolated. Work shall take place during normal business hours and Contractor shall abide by all VA safety rules and regulations and ensure work is in compliance with applicable NFPA codes. (2) With the installation of new piping from each chemical feed tank, boiler plant personnel will be able to easily isolate each boiler and provide the proper chemical treatment for it. Currently there are two chemical feed tanks that are piped through the DA (deaerator) tank and then to the boilers. To adjust the chemical treatment on a specific boiler, either additional or less chemicals have to be added in order to treat the specific boiler. This is not the best or efficient method to chemically treat the boilers and therefore each tank should be piped separately to each boiler. Once the two additional VA-provided chemical feed tanks are installed by the facility, Contractor shall perform the following work: o Remove the chemical feed piping to the existing DA tank. o Install new ¾" 304SS socket weld chemical lines from the discharge of the chemical feed pumps to each individual boiler. o Use existing trapeze hangers to the greatest extent possible to support the new chemical feed piping. New hangers shall be installed if not possible. o Furnish and install stop valves with injection quills. o Provide additional check valves, isolation valves, or unions as necessary for the chemical feed piping. Prior to starting this work, Contractor shall coordinate with Boiler Plant personnel to ensure that all necessary preparation work is complete. Contractor shall also verify prior to the start of work the piping route with VA boiler plant personnel and the tie-in points of the piping for each boiler. Work shall take place during normal business hours. Contractor shall abide by all VA safety rules and regulations and ensure work is in compliance with applicable NFPA codes. COMPLETION TIME (45) calendar days from NTP PRIVACY/SECURITY TRAINING REQ'MTS Prior to starting the construction project, all contractors and subcontractors, are required to complete VA Information Security and Privacy Awareness Course (VA10176) and submit a copy of the certificate to the COR. (See General Requirements for more details). GENERAL REQUIREMENTS o Prior to starting any work on-site, contractor along with all sub-contractors shall wear ID badges at all times. The ID procedure will be discussed at the pre-con meeting. o Contractor shall not take any photographs without the authority of the contracting officer in writing. Contractor along with VA personnel shall discuss work assignments and check-in check-out procedures at the pre-con meeting. o Report to Engineering to sign in. RECORDS AND INFORMATION MANAGEMENT Project No. 679-15-R02 Project Title: Boiler Plant Modifications/Upgrades Upon completion or termination of the applicable contract(s) or agreement(s), return and/or destroy, at Covered Entity's option, VA information gathered, created, received, or processed during the performance of the contract(s) or agreement(s). No data will be retained by Business Associate, or contractor, subcontractor, or other agent of Business Associate, unless retention is required by law and specifically permitted by Covered Entity. As deemed appropriate by and under the direction of Covered Entity, Business Associate shall provide written assurance that all VA information has been returned to Covered Entity or destroyed by Business Associate. If immediate return or destruction of all data is not possible, Business Associate shall notify Covered Entity and assure that all VA information retained will be safeguarded to prevent unauthorized Uses or Disclosures. Prior to termination or completion of this contract, contractor/subcontractor must not destroy information received from VA, or gathered/created by the contractor in the course of performing this contract without prior written approval by the VA. Any data destruction done on behalf of VA by a contractor/subcontractor must be done in accordance with National Archives and Records Administration (NARA) requirements as outlined in VA Directive 6300, Records and Information Management and its Handbook 6300.1 Records Management Procedures, applicable VA Records Control Schedules, and VA Handbook 6500.1, Electronic Media Sanitization. Self-certification by the contractor that the data destruction requirements above have been met must be sent to the VA Contracting Officer within 30 days of termination of the contract.
