- BLANK W-9 FORM MTC VENDOR SELF CERTIFICATION FORM MTC TERMS AND CONDITIONS DAVIS BACON WAGE DETERMINATION CONSTRUCTION BID PACKAGE (RFQ) All, this is a subcontracting opportunity with Management and Training Corporation (MTC), operator of the Flint Hills Job Corps Center (FHJCC). Please accept this as your invitation to attend our pre-bid meeting for the Administration Building Window Replacement Project at FHJCC located at 4620 Eureka Dr., Manhattan, KS 66503.　 The meeting will be conducted on June 23, 2015, @ 10:00 AM (CST) with bids due on Tuesday,　 July 28, 2015 @ 3:00 PM (CST). The attached "Construction Bid Package (CBP)" contains the requirements of the project. The Davis Bacon Wage Schedule, W-9, MTC terms and conditions,　 and MTC Vendor Self-Certification forms are provided for your convenience. All forms, references and bid breakout　 (if required - see Item #1 below) must be returned with our Bid for Lump Sum Contract (page 3 of the CBP) in order to be deemed responsive. Please pay close attention to the "Instructions to Bidder" as outlined below and part of the CBP. MTC reserves the right to reject any or all bids or waive any formality connected with the bids. MTC will be the sole judge of which bid is the best value and its determination and finding on such decision will be final. MTC may elect to award contract on a per deficiency basis.
