- Service for Annual Medical gases testing, vacuum pump and Medical air PM, Trace gases analysis and air exchanges. (I)This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested, and a written solicitation will not be issued. (II)This solicitation is issued as a Request for Quote (RFQ). Submit written quotes on RFQ VA246-15-Q-0848. (III)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-82. (IV)Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov). (V)The Network Contracting Office 6-SAO EAST intends to award a firm-fixed price service contract for Annual Medical gases testing, vacuum pump and Medical air PM, Trace gases analysis and air exchanges located at the Beckley VA Medical Center, 200 Veterans Avenue, Beckley, West Virginia 25801. (VI)The North American Industrial Classification System (NAICS) code for this procurement is 325120 with a large business size standard of 1000 employees. This procurement is unrestricted to and for all eligible offerors may submit quotes. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The record must be complete and not missing elements such as representations and certifications. (VII)Questions concerning this solicitation shall be addressed to Gail Bargaineer, Contracting Specialist, and emailed to Gail.Bargaineer@va.gov. All questions or inquires must be submitted no later than 11:00 AM Eastern Standard time on June 5, 2015. No phone calls will be accepted. (VIII)The Government will award a contract resulting from this RFQ to the responsible offeror whose quote is the lowest price and meets the requirements of the SOW. Quotes are due by 11:00 AM Eastern Standard time on an June 5, 2015. Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. (b)This is a base + 4 years option contract. Base year begins June 8, 2015 and ends June 7, 2016. The last option year expires June 7, 2020. (c)This is a semi-annual service that requires multi-day trips to complete preventive maintenance program, for medical air compressor and vacuum pump system, (d)This requires certified medical gas outlets/inlets evaluation technician, required technician familiar with medical gas outlets/inlets. 1.Complete inventory of all outlets/inlets will be produced 2.Area, floor & room number will be identified 3.Manufacturer/model of outlets/inlets will be identified 4.Flow rate & gas concentration will be tested on each available outlet 5.Flow rate will be tested on each available vacuum/WAGD inlet 6..45 micron filter sample will be collected at one outlet in each critical care zone 7..45 micron filter sample will be collected in 20% of non-critical care zones 8.Pressure drop will be tested once per zone 9.Average flow rate in each zone will be documented 10.Pressure or vacuum level will be documented on each zone 11.Mechanical integrity, adaptation will be verified on each available outlet and inlet 12.All leaks, missing parts or other relevant observations will be noted. (e) MAINTENANCE AND SERVICE (PM): Requires for a certified a preventive maintenance program, for medical air compressor and vacuum pump system, to be completed on a semi-annual schedule. (f) SOURCE EVALUATION: BULK MEDICAL OXYGEN SYSTEM: 1.Visually inspect bulk site 2.Compare system to current NFPA 99 & NFPA 55 standard 3.Capacity 4.Enclosure/Location 5.Check system components for leakage 6.Fire safety considerations 7.Provide documentation of all inspection results (g) MEDICAL AIR SYSTEMS 1.Visually inspect medical air system 2.Compare system to current NFPA 99 standard 3.Capacity 4.Visually observe system for proper operation 5.Document Manufacturer/Model information 6.Verify intake piping location 7.Check system components for leakage 8.Provide documentation of all inspection results (h) MEDICAL GAS MANIFOLDS 1.Visually inspect all medical gas supply manifolds 2.Compare manifold to current NFPA 99 standard 3.Enclosure/Location 4.Capacity 5.Document Manufacturer/Model information 6.Check cylinder connections for leakage 7.Check manifold components for leakage 8.Provide documentation of all inspection results (i)MEDICAL VACUUM SYSTEMS 1.Visually inspect vacuum system 2.Compare system to current NFPA 99 standard 3.Capacity 4.Visually observe system for proper operation 5.Document Manufacturer/Model information 6.Verify exhaust piping location 7.Provide documentation of all inspection results (j) MASTER ALARM PANELS 1.Locate each Master Alarm panel 2.List each individual existing signal as labeled on panel 3.Identify all required signals that are missing 4.Test button on each panel will be activated 5.Verify function of audible alert 6.Note any nonfunctional LED's or Lights 7.Document Manufacturer/Model information 8.Note number of blank modules & spare signals 9.Provide documentation of all inspection results (k)AREA ALARM PANELS 1.Locate each Area Alarm panel 2.Test button on each panel will be activated 3.Document manufacturer information 4.Note gas, pressures & area served as displayed on panel 5.Verify function of audible alert 6.Provide documentation of all inspection results (l)MEDICAL GAS ZONE VALVES 1.Locate each medical gas zone valve 2.Document manufacturer information 3.Note pressure displayed on gauge 4.Document valve type, size, gauge & indicated pressure 5.Valves will be checked for external leaks 6.Identify rooms served by each valve to the extent possible during the inspection process (e)Place of Performance: Tasks will be performed at the VAMC Beckley, WV. Location of work will be in the VAMC Beckley, WV. Work will be in an occupied healthcare facility in operation 24 hours a day and care must be provide to keep work areas clean and contained. The contractor will be responsible for storage of materials and disposal of all unused materials. The contractor will ensure that all sub-contractors meet and abide by all government regulations and requirements to include 10 hour OSHA Training. (f) Contract Administration: Notwithstanding the Contractor's responsibility for total management during the performance of this contract, the administration of the contract will require maximum coordination between the Government and the Contractor. The following individuals will be the Government's points of contact during the performance of this contract: 1. Contracting Officer:Angel Rhone Contract Specialist:Gail Bargaineer (g) Contracting Officer's Technical Representative (COR): To be designated at time of contract award. The COR shall be designated on the authority of the Contracting Officer at the time of contract award to monitor all technical aspects of the contract. In no event is the COR empowered to change any of the terms and conditions of the contract. Changes in any section of this contract shall be made only by the Contracting Officer pursuant to a properly executed modification. The types of actions within the purview of the COR's authority are to ensure that the Contractor performs the technical requirements of the contract, and to notify both the Contractor and the Contracting Officer of any deficiencies observed. A memorandum of designation shall be issued to the COR and a copy shall be sent to the Contractor at the time of contract award setting forth in full the responsibilities and limitations of the COR. The Contracting Officer may exercise the option by written notice to the Contractor within 60 days. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years.
