All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. REQUIREMENT The Department of Veterans Affairs has established a need for improved healthcare access in Altus, Oklahoma and surrounding counties in Oklahoma. In order to assess current market conditions, formulate an accurate cost estimate, and develop an effective Performance Work Statement, the Government is requesting industry feedback with respect to the areas listed below. Any information submitted by and obtained from industry as a result of this notice may be used in development of the final Performance Work Statement, cost estimates or terms and conditions of the actual solicitation. The Oklahoma City VAMC "the parent facility" requires CBOC services providing Primary Care and Tele-Health services in a private hospital, office or clinic environment to veterans, primarily residing in Altus, Oklahoma. Within the bounds of Altus, Oklahoma and/or surrounding counties, potential sources shall be able to provide primary care and mental health support services for eligible veterans living in those areas. The contractor shall furnish health care providers, medical facilities, equipment and supplies, emergency-fill pharmaceutical prescriptions, and administrative functions to fulfull the support of enrolled patients. The contractor shall be responsible for the administration and management of all aspects of this contract and the health plan covered there under, including responsibility for all employees, agents, subcontractors and any other entity acting for or on behalf of the contractor. VA requires access to the primary care provider within 20 minutes of the scheduled appointment time, within 7 calendar days of the veteran's request for an appointment for established patients and 14 days for new patients. Primary Care services and Mental Health support shall be provided to patients enrolled to the clinic by Oklahoma City VAMC. Payment for these services will be based on a monthly capitated rate. These services are defined in the Performance Work Statement in this contract. PLACE OF PERFORMANCE: The patient population treated at the VA Community Based Outpatient Clinic (CBOC) in Altus, Oklahoma or within a 30 mile radius; therefore, competition may restricted to the local region. The NAICS code to be utilized for this acquisition will be 621498 - Outpatient Care Centers - and the small business size standard applicable is $20.5M. Submit responses via email to Fredrick.Wilson@va.gov no later than 3:00pm central time June 9, 2015. Responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. Please clearly identify any information to be considered sensitive or proprietary. Respondents will not be notified of the results of the market research. Use keywords "Altus Market Research" in the subject line. Interested sources shall provide the following information in their response 1.Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s) and DUNS Number. Proof of Systems for Award Management Registration (SAM) is mandatory. If applicable, please include your GSA schedule, small business status and if applicable, proof as a Veteran Owned Small Business (VOSB) or Service Disabled Veteran Owned Small Business (SDVOSB). VOSB and SDVOSB eligibility must be verifiable in http://www.vetbiz.gov/. 2.A brief capability statement demonstrating past performance with relevant contract services in Prime Contractor role. Capability Statement shall not exceed more than 3 pages. Note: Do not include Proprietary, classified, confidential, or sensitive information in responses. Respondents should clearly describe their capabilities to include management experience, equipment and personnel or the ability to obtain them. Furthermore, interested sources must also meet all requirements of Federal, State, and Local Municipal codes regarding operations of this type service.
