Description

Contractor shall go out into the community to target hard to reach homeless Veterans in the Greater Los Angeles area through community outreach activities, identify eligible Veterans through point in time counts and engage other service providers to complete referrals through the local Coordinated Entry System. Once identified, Veterans will be thoroughly screened for admission into various VHA the programs such as HUD-VASH and referred to the VA Greater Los Angeles Healthcare System. It is understood that the contractor will screen more Veterans than meet admission criteria.
