- jeffries area maps quote doc and clauses duchars maps uniontown south maps This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested. Solicitation AG-52B1-S-15-0032 is being issued as a Request for Quotation (RFQ), to perform openings maintenance services on the Hoosier National Forest, Tell City Ranger District, in Perry County, Indiana. Work shall consist of bush-hogging unwanted woody species in project areas. This acquisition is set-aside for small businesses. The North American Industry Classification System (NAICS) code is 561730 and the size standard is $7.5mil. Work will not begin until August 15. Contract time is approximatly 75 days. Offerors shall provide both pricing and technical information. Pricing and technical information will be submitted to: Roger Manning, USDA Forest Service, 811 Constitution Ave, Bedford, IN 47421. NO FAX QUOTES WILL BE ACCEPTED. E-mail quotes are acceptable at rkmanning@fs.fed.us. Service Wage Rates are applicable. Quotes must include vendor's TIN (Tax Identification Number) and DUNS number. Vendors must be registered in SAMS (www.sams.gov) Quotes must include a completed copy of (FAR 52.212-3 Offeror Representations and Certifications, Commercial Items, paragraphs (c) through (m). If the Offeror has completed the annual representations and certifications electronically at http://orca.bpn. gov., submit only paragraph (b) from the provisions and a DUNS number. (FAR) 52.212-2 Evaluation, Commercial Items is applicable with the following factors used to evaluate quotes: Price, technical conformity to specifications, and past performance. Clauses indicated on a separate attachment. Quotes are due June 18, 2015 at 11:00a.m. Eastern Time. A pre-quote meeting will be Thursday, June 4 at 9:00a.m. Central Time at the Indian Celina Gatehouse.
