Request for Quotes with clauses Wage Rates Door County Wisconsin STURGEON BAY HOUSING 314 N. 16th PLACE SCOPE OF WORK FOR INTERIOR REMODELING FOR UPCOMING GSA OPEN HOUSE PART 1 PROJECT DESCRIPTION 1.1.1 The Coast Guard requires a contractor to provide all permits, labor, materials and equipment necessary to complete interior and exterior repairs due to components failing and exceeding their life expectancy. PART 2 SCHEDULING 2.1 All work associated with this project will be conducted between the hours of 8:00 AM through 5:00 PM, Monday through Friday. 2.2 If circumstances should arise wherein the contractor desires a different work schedule as required in section 2.1, the contractor must request that in writing. Submit written request to the Coast Guard representative DC2 Russell Poole a minimum of 24 hours before the requested adjusted work schedule. If the desired schedule change is to be effective on a Monday, the request must be submitted to the POC prior to 9:00am on the Friday prior to commencement of work. PART 3 PREPARATION 3.1 Contractor will be responsible for any permits that may be required by Wisconsin state law and the City of Sturgeon Bay, WI. PART 4 MATERIALS 4.1 Government furnished materials: The Coast Guard will not provide any materials. The contractor shall supply all materials. 4.2 Contractor Supplied Materials: The contractor shall provide all materials necessary for completion of the job. PART 5 APPLICATION 5.1 All work performed will be in compliance with all applicable Federal, State and City codes. PART 6 CLEAN UP 6.1 Contractor shall ensure area is left in a clean and orderly condition. 6.2 Upon completion of this project the Coast Guard representative DC2 Russell Poole will inspect all work before any payment is received. PART 7 CONTRACTOR PERFORMANCE SPECIFICATIONS 7.1 All work shall be accomplished in a professional manner and in strict adherence to current manufacturer standards and recommendations for general construction. 7.2 Contractor shall adhere to the most current building codes, city/state ordinances and OSHA safety regulations at all times. 7.3 Contractor will: a. Remove and dispose of the front and side exterior entry doors. b. Install new front entry door meeting the following minimum standard: white painted door on sides, new lock set and deadbolt, new trim. c. Install new side entry door meeting the following minimum standard: white painted door on sides, new lock set and deadbolt, new trim. d. Repair damage to the front concrete step. e. Install new baseboard around front door (stained and varnished). f. Repair damaged drywall board in bathroom ceiling; install new bath room vent fan, prime and paint entire bathroom ceiling. g. Any issue that is discovered while work is being conducted and that is not directly addressed in this scope of work (condition found issues) will be brought to the attention of the Sturgeon Bay Housing Petty Officer DC2 Russell Poole and the BASE Cleveland OHMC DCC John P. Treme' before proceeding with the project beyond the point of discovery. PART 8 QUALITY ASSURANCE 8.1 Any work that does not meet the approval of the U.S. Coast Guard representative shall be immediately corrected. Unsatisfactory work shall be redone at the contractor's expense. Once all work is completed the Coast Guard representative will sign off on the project as completed. PART 9 LIABLITY INSURANCE 9.1 It is the responsibility of the contractor to provide their own liability insurance for contractor and employees. In no case will the Coast Guard assume liability for bodily injury to the contractor and/or employees. PART 10 GOVERNMENT FURNISHED SERVICES 10.1 The government will provide the following services for the contractors use during the completion of this project: water, electricity and lavatory. PART 11 LABORER WAGES 11.1 Clause 52.222-6 Davis Bacon Act is incorporated DOL Wage Rates can be found at www.dol.gov. This contract will require contractors to pay their laborers and mechanics employed under this contract no less than the locally prevailing wage and fringe benefits for the corresponding work on similar projects in the area.
