Description

STATEMENT OF WORK A.GENERAL INFORMATION 1. Title of Project: Transvenous Intrahepatic Portosystemic Shunt Procedures requiring a covered stent for new services offered in Interventional Radiology. 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The lined portion stent should be able to cover the entire tract creation from the liver parenchyma, portal vein junction to the hepatic vein ostium. 3. Background: Effectively treating portal hypertension in both the creation and revision of TIPS procedures. All prior TIPS procedures ordered at the Atlanta VA have been fee- based out to other facilities, however due to the new circumstances such as a new credentialed Physician and the anesthesia compliant Interventional Radiology procedure suite, all TIPS procedures are a newly offered service. 4. Period of Performance: 2015- 5. Type of Contract: Firm Fixed Price B.DELIVERY LOCATION: ATTN: Brackita Washington Logistics Warehouse, Veterans Affairs Medical Center, 1670 Clairmont Rd. Decatur, Ga. 30033 C.SCHEDULE OF DELIVERABLES/INSTALLATION Supplies will delivered upon purchase to Logistics warehouse, Attn: Brackita Washington. Upon receipt we will deliver to Interventional Radiology department. This is to be a one-time initial purchase, and the product will effectively be replaced upon use through the consultation process that is currently in use by the Prosthetics department.
