- Brand Name Justification -- Redacted RFQ Price Sheet RFQ Clauses and Provisions This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 and 13.106-1 (d), as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-81. The Washington Headquarters Services, Acquisition Directorate (WHS AD), is requesting a quote for the following Laboratory Supplies on a brand name or equal basis. Contractor shall specify items being proposed in accordance with 52.211-6 Brand Name or Equal (Aug 1999). Brand Name only items are indicated within the attached supply list. This solicitation is a total set-aside for small businesses associated with the North American Industrial Classification System (NAICS) code 423490, with a business size standard of 100 employees. This item is being solicited on the open market, however, please identify any items that may be procured from your GSA contract vehicle. The government reserves the right to combine both open market and FSS into one contract for administrative convenience. In the event this occurs, items awarded will be subject to FAR part 8 for FSS and 12 and 13, for open market items. Clauses and Provisions associated with this RFQ can be found in Attachment 1. A price sheet has been included and is to be used when submitting quotes in response to this RFQ. Please see price sheet in Attachment 2. The required FAR 13 brand name approval is attached as Attachment 3. The end user is the Pentagon Force Protection Agency (PFPA)/Chemical, Biological, Radiological, Nuclear and Explosives (CBRNE). DELIVERIES : The Contractor shall ship the requested lab consumable supply items from the CBRNE Laboratory Supply List. Delivery is to be Destination. The requested delivery date is 45 days after the date of contract award (ADC). Any shipping costs should be included in the total cost to deliver the supplies. RETURNS/ORDER ERRORS : The Contractor shall pay for the return of any item(s) shipped in error to the PFPA. The correct items shall be shipped to the Government within seventy-two (72) hours of notification of the error. The Contractor shall notify the Technical Point of Contact as soon as an error has been discovered. BREAKAGE : Any item received broken or in poor condition from the contractor to PFPA, shall be the responsibility of the contractor to replace free of charge to the Government which includes any shipping and/or handling charges. Please include "CBRNE Laboratory Supplies RFQ" in the subject line for all questions and for submission of quotes. Vendors are advised to submit electronic documents early and confirm successful transmission/receipt. No phone calls please. All quotes shall be submitted via email to the POC stated below. Quotes must be for the items requested or their equal unless indicated as a "Brand Name Only" item and as such shall not be substituted. All quotes are due by 12:00 NOON ET, May 16, 2015. Award will be made based on the lowest priced, technically acceptable quote for the commercial items and is required on an all or none basis, no partial quotes will be accepted. An official authorized to bind your company shall sign and date the quote. No late questions or quotes will be accepted.
