Attachment 3 - Past Performance Questionnaire Attachment 2 - Schedule of Services Attachment 1 - Performance Work Statement This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for which quotes are being requested and a written solicitation will not be issued. The solicitation number is FA4486-15-MG001 and is issued as a request for quotation. This solicitation document and incorporated provision and clauses are those in effect through Federal Acquisition Circular FAC 2005-82. The associated North American Industrial Classification System (NAICS) code for this procurement is 561720. This requirement is full and open competition. Period of performance is one basic year plus four one year options, 1 October 2015 through 30 September 2020. Offerors must be registered in the System for Award Management (SAM). If the company is not registered they may do so by going to https://www.sam.gov All quotes are due no later than 18 June 2015 at 16:00 Local, Azores, Portugal Standard Time. Quotes may be delivered to the following address: 65th Contracting Flight, Avenida do Imperio, Bldg. T-615, Lajes Field Azores 9760-277, or may be sent via e-mail to maria.godinho.pt@us.af.mil Description of Service: The contractor shall provide all management, labor, tools, equipment, materials, and transportation necessary to clean interior and exterior surfaces of kitchen hood and exhaust systems located in the facilities listed on the performance work statement (attached), located at Lajes Field, Azores, Portugal. The following provisions apply to this acquisition: 52.212-1, Instructions to Offerors -- Commercial, and addenda included below; offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The following clauses apply to this acquisition: 52.212-4, Contract Terms and Conditions -- Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items. 5352.215-9001 - Notice of Pre-Proposal Conference A pre-proposal conference/site visit will be conducted at the 65th Contracting Flight, Bldg. T-615 2nd Floor on 2 June 2015, starting at 10:00 AM (local Azores Time). All prospective offerors are highly encouraged to attend. A maximum of 3 people per company will be allowed to attend. Offerors that will require a Base pass shall provide the name and citizen identification number or passport number of company representatives to maria.godinho.pt@us.af.mil no later than 1 June 2015, 12:00 AM (local Azores time) Addenda to 52.212-1 - Instructions to Offerors -- Commercial Items (April 2014) (b) Offers must be submitted in accordance with the instructions contained in the Provision 52.212-1 - Instructions to Offerors - Commercial Items and this addenda. (i) Price. Provide pricing per CLIN on the attached schedule of services (Attach. 2) Pricing shall clearly specify the currency used (US Dollars or Euro). Quotes shall be valid until 31 October 2015. (ii) Commercial Service Permit. Provide a copy of your permit (Certidao Commercial), related to this service. (iii) Past Performance. Offerors must receive an Acceptable rating to be eligible for an award. This rating is based upon the past performance questionnaires received and any information obtained from other government sources of past performance information. Offerors shall forward a copy of the past performance questionnaire (Attach. 3) to a maximum of four (4) agencies whom the offeror has been contracted for similar services with the Federal State, local Government or from private industry sources during the past three (3) years. The past performance acceptable criteria below will be used to evaluate proposals: Acceptable: Based on the offeror's performance record, the government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror's performance record is unknown. (See note below.) Unacceptable: Based on the offeror's performance record, the government has no reasonable expectation that the offeror's will be able to successfully perform the required effort. Note: In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably. The offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, "Unknown" shall be considered "Acceptable." (End of Addenda) 52.212-2 -- Evaluation -- Commercial Items (Oct 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) Price (ii) Commercial Service Permit (Certidao Commercial) (iii) Past Performance (End of Provision) 52.217-8 -- Option to Extend Services (Nov 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within thirty (30) calendar days. (End of Clause) 52.217-9 -- Option to Extend the Term of the Contract (Mar 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within thirty (30) calendar days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least sixty (60) days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years and six (6) months.
