- wage rates construction clause SCOPE OF WORK PORT CLINTON HOUSING ASBESTOS ABATEMENT 1. WORK COVERED BY CONTRACT: • The asbestos abatement contractor shall remove all asbestos containing material (ACM) contained in the textured ceilings, floor time and mastic throughout the following two homes: 912 Grant Street (1,377 estimated square foot) and 1008 Grant Street (1,624 estimated square foot), Port Clinton, OH. 2. DESCRIPTION OF WORK TO BE COMPLETED: • The asbestos abatement contractor shall supply all labor, materials, services, permits, equipment, utility and transportation services required and necessary for the removal, transportation and disposal of asbestos containing materials and asbestos contaminated materials throughout both homes. The location of know ACM is the textured ceilings, floor tiles and associated mastic throughout all rooms. • All procedures and equipment used must provide containment of asbestos debris and not allow airborne contamination of adjacent areas. • The Contractor shall provide the procedure to be followed for handling asbestos waste on removal, packaging, transport and disposal to ensure they comply with the requirements of Code of Federal Regulations 40, Part 61, Subparts A and M. • The Contractor shall submit material safety data sheets (MSDS) and manufacturers literature for all products he intends to use on this project. • The final step should be to employ thorough cleanup procedures to properly control the ACM. Contractor shall conduct a careful visual inspection and final clearance air monitoring to verify satisfactory cleanup. 3. CONTRACTOR QUALIFICATIONS: • The Coast Guard may accept those contractors as being qualified if have met all of the licensing requirements of Federal, State and Local code/law associated with asbestos abatement in the State of Ohio. Contractor shall provide evidence of certification for the following : o Asbestos Contractor License o All workers must be certified as Asbestos Hazard Abatement Workers (AHAW) or as Asbestos Hazard Abatement Specialists (AHAS) o Asbestos hazard evaluation specialist o Asbestos hazard project designer o Hazard abatement air-monitoring technician 4. SITE VISIT: • Those who desire access to the homes for bidding or work are to contact DC2 Vernon Horst. Once the contract has been awarded, contractors are to arrange work hours and work days needed to complete this project through the U.S. Coast Guard Contracting Officer and the housing office point of contact DC2 Vernon Horst. 5. MILITARY REGULATION: • The contractor, his employees and sub contractors must obey all unit regulations. • All personnel employed on the project shall keep within the limits of the work and avenues of ingress and egress. Personnel shall not enter any other areas outside the site of work unless required to do so in the performance of their duties and have obtained verbal permission from DC2 Vernon Horst or the Base Cleveland Owned Housing Maintenance Coordinator, DCC John Treme. • The contractor's equipment shall be conspicuously marked for identification. • There shall be no smoking in any Coast Guard home and only in designated areas previously agreed upon by the contractor and DC2 Vernon Horst. 6. WORK HOURS: • The contractor shall perform all site work during the normal operation hours, 0700-1600 Mon-Fri except during federal holidays. • Any deviations from normal working hours will be approved by DC2 Vernon Horst and DCC John Treme. 7. CLEAN UP OF WORK AREA: • The contractor shall remove and properly dispose of all trash, debris and ACM pertaining to the contract work from the limits of government property, as well as adjacent affected areas ensuring they are in compliance with EPA regulations. The contractor shall ensure that all areas of the home impacted by this contract are properly cleaned using wet cleaning, wet wiping practices along with the use of HEPA vacuums. 8. ACCEPTANCE OF WORK: • Prior to the acceptance of work by the Coast Guard the contractor shall arrange through DC2 Vernon Horst a review of work completed to ensure work satisfaction as well as proper clean up. 9.
