- Attachment 5 - Past Performance Assessment Questionnaire Attachment 4 - Request for Past Performance Information Evaluation Attachment 3 - Client Authorization Letter Sample Attachment 2 - Past Performance History Attachment 1 - Statement of Objective (SOO) RFP FA8052-15-R-0007 (Conformed Copy) Solicitation Questions: Questions relating to the synopsis/solicitation and /or terms and conditions shall be submitted in writing by email or fax only to individuals identified herein NLT 1:00 PM, Local San Antonio time on Thursday, 28 May 2015. Any questions received after this time/date may not be answered due to time constraints of the solicitation. Proposals are due NLT 11:00 AM, local San Antonio TX time on Thursday, 11 June 2015. Estimated award date is 23 July 2015 with performance to begin 01 August 2015. The resulting award will be Firm-Fixed (FFP) contract, with a twelve (12) month base period and four (4) - one year options, if all options are exercised. The Period of Performance (PoP) is scheduled to begin 01 August 2015. Attached is the RFP FA8052-15-R-0007 and attachments one (1) thru five (5).
