- THIS IS NOT A SOLICITATION NOTICE This is a market survey for information and planning purposes only. This Request for Information (RFI) serves to improve the understanding of Government requirements and industry capabilities regarding Home Oxygen Services and allows potential offerors to judge whether or not the Government's requirements can be satisfied within offerors' capabilities. Funding is not available at this time. North American Industry Classification System (NAICS) code: 532291. BACKGROUND The Orlando Veteran Affairs Medical Center (VAMC), 5201 Raymond Street, Orlando, FL 32803 has a requirement for home oxygen services for approximately 1200 veterans that are served by the Central Florida Healthcare System (CFHCS). All services shall be provided either at the VA Medical Center, the veteran's residence, or in a nursing home where the veteran resides. The Contractor shall have a physical location which houses the service office, warehouse, dispatch of vehicles and other functions related to the performance of services of this contract within a fifty (50) mile radius of the Orlando VAMC to facilitate Joint Commission and/or VA inspections. Services provided shall be all inclusive of deliveries, transportation, and delivery/set-up of equipment, in-service training for beneficiaries/care providers to include patient education, safety inspections and preventive maintenance inspections (PMI) service appointments. Anticipated performance period is FY 2015 - FY 2016 SMALL BUSINESS DETERMINATION In order for the Government to set aside any acquisition over the Simplified Acquisition Threshold ($150,000), there must be a reasonable expectation that offerors can satisfactorily perform contract requirements and that award will be made at a fair market price. Total set-asides cannot be made unless such a reasonable expectation exists. The Government believes that it is mutually beneficial to determine the capability of offerors eligible for set-asides early in the procurement process in order to preclude the need for a modification of the acquisition strategy later. Your response will assist greatly in the formulation of an acquisition strategy that maximizes the participation of small business, veteran-owned small business, service-disabled veteran-owned small business, HUBZone small business, small disadvantaged business or women-owned small business concerns. Interested firms are asked to respond to RFI with your experience and capabilities for home oxygen services. VENDOR CAPABILITY Interested and qualified vendors are encouraged to: 1) Send a summary of their company's capabilities, and company history/information, license to operate in Central Florida (Orange, Seminole, Volusia, Brevard, Osceola, among others) and any information you deem beneficial to this RFI. 2)Describe capability to meet requirement of Non-Manufacturing Rule (FAR 19.102 (4)) for equipment (i.e., nasal cannula, tubing, cylinder w/oxygen regulator, cylinder handcart, etc.). 3)Include your company's point of contact, address, e-mail address, telephone, DUNS, Socio-economic identification (WOSB, SDVOSB, VOSB, etc.).
