Description
STATEMENT OF WORK FOR TREE REMOVAL AND TRIMMING VA MEDICAL CENTER WEST HAVEN, CT 06516 1.SCOPE: The work of this project is for tree and branch removal and tree trimming located throughout the VA Medical Center, West Haven, CT. There are a variety of tree types and calipers. The work includes, but is not limited to providing all labor, supervision, quality control, safety prevention, tools, materials, equipment and transportation necessary to provide Tree Trimming Services. 2.REQUIREMENTS: The Contractor shall schedule all work in advance with the Contracting Officer's Representative (COR). Mandatory walk through to ensure a complete understanding of the project. Work shall be performed by a Connecticut Licensed Arborist. The Contractor shall provide qualified supervision at all times when work is being conducted by Contractor employees. The supervisor shall maintain daily reports of all work and work conditions. Copies of daily reports shall be accessible to the Contracting Officer. Where schedule requires removal of a tree, the stump shall be removed or ground to below grade. Any areas disturbed by construction or equipment shall be restored to match the condition and material that existed prior to the start of work. All wood and debris shall be removed by the Contractor and disposed of. All work areas shall be raked and cleaned upon completion. 3.SAFETY: Contractor shall maintain all safety requirements for tree work including but not restricted to all personal protective gear and safety regulations regarding the use of a boom truck, chipper, power saws or any other machinery. Contractor shall at all times protect the residents and their property from harm. The Contractor shall take all necessary precautions to meet proper safety standards and regulations of the State and Federal Codes. OSHA regulations apply to all work performed on VA Medical center property. LOCATIONS/AREAS/TYPE OF TREES: 1. Front Entrance/Campbell Avenue: Right side of front entrance a. 9 Potentially hazardous trees impacting service lines b. Trim back laterals over street on Oak tree on slope 2. Top of slope/Campbell Avenue entrance/approx. halfway up drive a. Elm (cut down) b. Ailanthus (cut down) c. 3 potentially hazardous white pines 3. Left side of entrance between new retaining wall and parking areas a. 9 small trees impacted by recent construction; including scotch pine growing into large Oak b.Remove dead limbs from Oak (Building #9) 4. Left front side of Building #12 a.Leaning Oak b.Hemlock 5. Rear of Building # 14 on slope a.Maple b.Oak c.Oak d.Oak e.Cherry (left rear of building) f.Austrian Pine (front between Campbell ave fence and lot) g.Two large Shadblow on slope h.Remove limbs on 9 Oaks and 2 White Pines (Rear of Building #14, by Spring Street fence) i.Remove suckers for 40" Oak (rear of building #14) 6. Across from Building #36/far north west corner a. 2 Leaning Red Maples (by fence to right of temporary office) b.Oak (right of blue container) c.3 Oaks (left entrance area) d.1 Black Oak and 2 white Oaks, trim to reduce wind resistance, raise tips 12' over lawn/parking area (rear parking lot edge) e.Raise all lateral limbs over 150' stretch of parking spaces (along north-west property border fence) f.Take down approx.. 30 small trees growing near or out of fence line (far north corner) 7. Building # 3 (rear) a. Clear Ailanthus/Mulberry tips 4-6' off Building #3 Roof 8. Buildings # 8 and #9 a. Remove large dead, diseased, crossing limbs and suckers from large Oak in center of front drive circle. b.2 stressed crabapples on lawn of Building #8 9. Service area parking lot/second left off Main entrance, then first right a. Decaying Maple b. Maple by driveway, cement in leader (by blue container) c.Remove approx. 8-10 large dead limps from 9 Oaks on lawn d.Remove lower and upper limps over portable office roof 10. Spring Street Side a. Remove deadwood from 2 Norway Maples (along retaining wall); raise 10-12' over driveway b.Decaying Maple (left side of Spring Street entrance)
