- RFQ NOTICE CONCERNING REQUEST FOR QUOTATION PHMSA RFQ# DTPH5615R00003 "Performing Research on Firework Chains" You are invited to submit a quote in response to PHMSA's Request for Quotation (RFQ) #DTPH5615R00003. Please see the attached Statement of Work (SOW) for the description of the services/supplies to be purchased under this RFQ. Also, please read this document in its entirety as it contains pertinent information (quote contents, evaluation factors, due dates, etc.) regarding this RFQ. ELIGIBILITY: 1. Quoter must hold an approval from the DOT, PHMSA Associate Administrator of Office of Hazardous Materials Safety for the purposes of explosives classification testing, meeting all requirements in 49 CFR Section 173.56(b). Quoter must provide a copy of the approval letter from the DOT, PHMSA Associate Administrator of Office of Hazardous Materials Safety with the quote. The 49 CFR Section 173.56(b) can be found at the following location: http://www.ecfr.gov/cgi-bin/text-idx?rgn=div8&node=49:2.1.1.3.9.3.25.7. A list of entities that currently hold the approval can be found at the following location: http://www.phmsa.dot.gov/hazmat/regs/sp-a/approvals/explo-test-labs. I. CONTRACT TYPE AND AWARD PHMSA intends to award a Firm Fixed-Price Purchase Order under FAR Part 12 and 13.5.  Applicable NAICS code: 541712  (Anticipated) Period of Performance: 1 year from the effective date of award. Award will be made to the responsible quoter that represents the best value to the Government, using a tradeoff process based on the Evaluation Criteria in Section III below. II. QUOTES CONTENTS Quotes must reference PHMSA RFQ# DTPH5615R00003 and must consist of Quoter's Tax Identification Number, DUNS, point of contact information, and proposal date should be clearly indicated on the cover page. Please send quotes via email to Mr. Shaukat Mirza ( Shaukat.Mirza@dot.gov ) with a copy to Mr. Warren Osterberg (warren.osterberg@dot.gov ), as well as the following: 1. Signed SF 18 form. 2. Signed Amendments, if any are issued. 3. Technical Capabilities: a. A narrative section titled Technical Approach on how the quoter will manage the requirements as stated in the SOW. (Limit: 15 pages total, not including Cover Page, Table of Contents, and pricing. Additional pages beyond 15 will not be evaluated. b. In section titled Technical Approach the quoter must hold an approval from the DOT, PHMSA Associate Administrator of the Office of Hazardous Materials Safety for the purposes of explosives classification testing, meeting all requirements in 49 CFR Section 173.56(b), which illustrates the necessary qualifications as stated in the SOW. Quoter must provide a copy of the approval letter from the DOT, PHMSA Associate Administrator of Office of Hazardous Materials Safety with the quote. A section titled Past Performance stating the similar work within the past three (3) years, as described in the SOW. 4. Pricing will be a separate page, and will be based on the requirements as stated in the SOW. III. EVALUATION OF QUOTES All quotes received will be reviewed by the Acquisition Services Division (PHA-30) prior to being forwarded to the Program Office for technical review. This pre-evaluation review will be to determine that the quotes received comply with the instructions in the RFQ and contains all the necessary elements required for review. ** INCOMPLETE QUOTES WILL BE DETERMINED "NON-RESPONSIVE" AND WILL NOT BE EVALUATED. ** Award will be made to the responsible quoter whose proposal presents the best overall value to the Government, using a tradeoff process. PHMSA will evaluate and rank each proposal according to Factors 1 through 3 below, which are of equal importance. PHMSA will also evaluate the price of each proposal, but will not rank or score this factor. 1. Evaluation Criteria 1: Quoter must hold an approval from the DOT, PHMSA Associate Administrator of the Office of Hazardous Materials Safety for the purposes of explosives classification testing, meeting all requirements in 49 CFR Section 173.56(b), which illustrates the necessary qualifications as stated in the SOW. Quoter must provide a copy of the approval letter from the DOT, PHMSA Associate Administrator of Office of Hazardous Materials Safety with the quote. Evaluation Criteria 2: Technical and Management Approach: The Quoter demonstrates the understanding of the requirements, the reasonableness, logic, and feasibility of the approach, and the availability of dedicated resources to fulfill the requirements. 3. Evaluation Criteria 3. Price: Under this factor, PHMSA will evaluate the fairness and reasonableness of the quoter's proposed total price, including the reasonableness of the proposed level of effort and labor mix. Although this factor will not be scored, it will be a consideration in determining which proposal presents the overall best value to the Government. Order of Importance: Evaluation Criterias 1 and 2 are of equal importance. When combined, Factors 1 and 2 are significantly more important than Factor 3 (Price). IV. QUOTER RESPONSIBILITY DETERMINATION To be eligible for award, the Contracting Officer must determine that the Quoter is responsible according to the standards of FAR 9.1, "Responsible Prospective Quoters." PHMSA Contracting Officer will determine quoter responsibility by reviewing SAM and FAPIIS to ensure the prospective awardee is not excluded from federal procurement and is current on all federal taxes. V. QUESTIONS All questions regarding this RFQ must be emailed to Mr. Shaukat Mirza (Shaukat.Mirza@dot.gov) with a copy to Mr. Warren Osterberg (warren.osterberg@dot.gov). Do not contact any other PHMSA personnel during the RFQ stage. All questions must be received on or before Wednesday, May 13, 2015 at 12:00 pm (noon), Eastern Time. VI. SUBMISSION OF QUOTES Please send quotes via email to Mr. Shaukat Mirza (Shaukat.Mirza@dot.gov) with a copy to Mr. Warren Osterberg (warren.osterberg@dot.gov). Quotes must be received on or before Friday, May 22, 2015 12:00 pm (noon), Eastern Time.
