PAY SCHEDULE DRAFT SUBCONTRACT PAY SCHEDULE SCA WAGES EXC PAY SCHEDULE FAR FLOW DOWN REPS & CERTS VENDOR REGISTRATION INSTRUCTIONS TO BIDDERS Gainesville Job Corps Center Minimum Qualifications: Active, unrestricted license to practice in the State of Florida; Primary Care Physician preferred - family medicine, pediatrics, or internal medicine Board certification preferred Current DEA registration Scope of Medical Services: 1. The services of the Professional will be made available to the Center at the Center's medical facilities in Gainesville, Florida as follows: a. Such services will be provided not to exceed 6 hours per week, Monday through Friday as mutually agreed upon, to include emergency on-call and telephone consultation services. Holidays are excluded. b. The Professional will be fully supported by the Center's in-house medical staff and resources in providing the services for: i. Physical examinations of new students in order to establish a medical assessment record for each; ii. Interpretation of all routine skin (PPD), X-ray (when indicated) and laboratory test; iii. Treatment of student "Sick Call" cases; iv. Treatment and/or consultation of emergency accident situations; v. Provide or arrange for necessary immunizations as directed; vi. Provide for all necessary referrals for specialists and institutional care vii. Provide for continuing care of all hospitalized Job Corps students (with some exceptions) viii. Provide for twenty-four (24) hour emergency coverage; ix. Provide oversight of the Center's Family planning Program. 2. The Professional may be called upon to offer "emergency" treatment or telephonic advice in the case of extreme medical emergencies at hours of the day other than those assigned in Paragraph 1 above. 3. Services shall be available: a. At the Center b. At the Professional's Office c. At such other places as directed by the Center director and the Professional to meet the needs of the student. FLUOR PROPRIETARY INFORMATION 4. Monitoring of the total Center health Program to include: a. Advice to the Center Director and the Health Services Supervisor on all medical and health related issues. b. Provide for a comprehensive set of "standing orders" updated annually. c. Provide assistance to the Health Services Supervisor as required. d. Monitoring of the Center's Health Education Program. e. Ensuring the maintenance and confidentiality of all medical records. f. Provide assistance to the Health Services Supervisor in developing agreements with hospitals for inpatient care and other needed services. g. Ensure that specialty care is timely and delivered at a reasonable cost, h. Oversight of the Center's Family Planning Program. Option Pricing: Physician Services for this Center allow for one (1) Nurse Practitioner or Physician's Assistant. Such services will be provided not to exceed 6 hours per week The Physician may employ a Nurse Practitioner or Physician's Assistant. The Physician must also certify that the employee is covered under a Workman's Compensation Program and hold an active license to practice in the State of Florida. Note: Please review and utilize the attached Senior Executive Pay schedule. GJCC is a federally‐funded residential educational facility serving roughly 300 area youth in vocational, academic, and employment training & education services. Fluor Federal Solutions, LLC. (Fluor) invites you to provide your individual Firm-Fixed-Price bid for this proposal, in accordance with the Instructions to Offerors. This is a Request for Proposal (RFP) and does not commit any funds for procurement.
