- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Pursuant to procedures prescribed in Part 12, Acquisition of Commercial Items, and Part 13.5 Test Program for Certain Commercial Items. Solicitation W9127Q-15-T-0012 for non-personal services is being issued as a Request for Quotation. The RFP document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-80, effective 02 Mar 2015. This solicitation is unrestricted. The NAICS code is 621111. The small business size standard is $11.0 million dollars. The USPFO for Mississippi, MS National Guard intends to establish a Blanket Purchase Agreement (BPA) with one or more medical providers to provide OSHA physical examinations for members on the Civil Support Team who serve as the full-time Chemical, Biological, Radiological, Nuclear, Explosives and Natural Disasters response element for the Mississippi National Guard and other occasions as needed throughout the life time of the BPA. The resultant multiple awards will be a firm-fixed priced (FFP) blanket purchase agreement (BPA), with a performance period of five (5) years. The Government intends to make an award without discussion therefore; the initial proposal shall contain the offeror's best price and shall be determined to be the overall best value and most advantageous to the Government. Be advised that all interested parties must be registered in the System for Award Management (SAM) and Wide Area Workflow (WAWF). Information concerning SAM and WAWF registration requirements may be viewed via the Internet at https://www.sam.gov/portal/public/SAM/ and https://wawf.eb.mil/. All vendors will be required to submit invoicing through the Wide Area Work Flow electronic invoicing (WAWF). The Government intends to enter into a BPA for one base year (1 May 2015 to 30 April 2016) with four (4) option years (2020). This time can be shortened at the government's discretion. This BPA will have a period of performance of five (5) years from date of agreement via issuance of Blanket Purchase Agreement Calls. The BPA will be reviewed annually. OSHA physicals for the 47th Civil Support Team Requirements: 1. OSHA Medical examinations requirements will be per the attached PWS and BPA Agreement. 2. Place of Performance: Performance will take place within a 25 mile radius from 47th CST Unit address, 138 Military Drive, Flowood, MS 39232 and will be identified per BPA Call. 3. Quotes should include pricing per exam. Variance from the price list should be coordinated with the Purchasing and Contracting office prior to acceptance. Please note that a BPA is not a contract. A BPA is a streamlined method for acquiring commodities / services on an as-needed basis from qualified sources that are readily available and thus eliminate the need for repetitive costly re-procurements. There is no minimum or maximum guaranteed quantity. All BPA Call Orders must be acknowledged by the selected vendor in order to be retained on the BPA calling list. The Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this notice.
