Revised Statement of Work Revised Schedule of Items Table SHREDDING SERVICES DFAS CLEVELAND REQUEST FOR QUOTES (RFQ) NO: HQ0423-15-T-0014 This is a combined synopsis/solicitation (RFQ) to obtain services for collection, shredding/destruction and disposal of Government paper/material on an as needed basis at Defense Finance and Account Service (DFAS) Cleveland, OH in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This will be a firm fixed-price contract type. NAICS code for this announcement is 561990 with small business size standard $11.0 million. SOLICITATION CLOSING DATE : All quotes must be received by Noon (EST), Tuesday, March 24, 2015. Preferred delivery method for submission of your quote is via email: carrie.e.fairbanks.civ@mail.mil. Quotes shall include at a minimum the following: • Unit price for each line item for the base year and three option year periods as described in the Price Schedule section below; • Vendor Contact Name, Phone Number and Email Address; • Company Cage Code and/or DUNS Number; • Tax Identification Number; • Business Size: Small/Large/Other: _______________ Note: A Schedule of Items table has been provided as a separate attachment to assist with supplying the required pricing and vendor contact information. DESCRIPTION OF REQUIREMENT : Defense Finance and Accounting Service, Cleveland (DFAS-CL) requires services for collection, shredding/destruction and disposal of Government paper/material on-site and off-site for multiple locations as described in the Statement of Work (SOW) (attached separately). Price Schedule: Base Year : Collection, shredding/destruction and disposal of Government paper/material on an as needed basis as follows: (Note: a Bin is defined as either a 2 wheeled upright locking bin approximately 26"Wx46"Hx33"D or a 4 wheeled horizontal locking bin approximately 29.5"Wx37"Hx44"D each similar to a capacity of a 96 gallon tote.) CLIN 0001 Site 1: Anthony J. Celebrezze Federal Building. Estimated quantity for pickup on an as needed basis (estimated weekly) for paper and related items is 20 - 25bins. Estimated quantity for pickup on an as needed basis (estimated semi-annually) for diskettes and related items is 2 bins (estimated 2,000 diskettes/bin). Unit price shall be provided per month. CLIN 0002: Site 2: Isaac I. Campbell (Bratenahl). Estimated quantity for pickup on an as needed basis (estimated every other week) for paper and related items is 2 - 4 bins. Unit price shall be provided per month. CLIN 0003: Government Transported Media Bins to Contractor Facility. Estimated quantity for destruction at contractor's facitily on an as needed based (estimated 2 times per year) for media related material is 2 bins. Unit price shall be provided per bin. Celebrezze Federal Building, 1240 East 9th Street, Cleveland, OH 44199. Site 2- DFAS-Cleveland, Isaac I. Campbell (Bratenahl), 555 East 88th, Cleveland, OH 44108 Period of performance will be one (1) twelve (12) month base period and three (3) twelve (12) month option periods. The Government will award a single contract resulting from this announcement to the responsible quoter whose quote conforming to the solicitation will be most advantageous to the Government and provide the best price. Reminder: A table has been included as an attachment titled Schedule of Items Table to assist with submission of pricing for your convenience. INVOICING AND PAYMENT : Invoices shall be submitted as a 2-in-1 or Combo invoice via Wide Area Workflow (WAWF) to a WAWF acceptor that will be identified in the resulting contract per DFARS Clause 252.323-7006 Wide Area Work Flow. Invoices shall include the contract number, invoice number, total pounds of paper/material shredded and date of pickup. It is the Contractor's responsibility to register in the WAWF system in order to insure prompt payment. Once the invoice is received in WAWF, the Acceptor will review and if correct accept it for payment. WAGE DETERMINATION : The Service Contract Act of 1965, as amended, applies to this acquisition, per the Federal Acquisition Regulation (FAR) clauses that have been incorporated by reference (FAR 52.212-5). Wage Determination No 2005-2415, Revision No 14, dated 12/22/2014 is applicable to this solicitation and may be viewed at http://www.wdol.gov/. Pursuant to FAR 52.222-42, Statement of Equivalent Rates for Federal Hires, the following data is provided for informational purposes only and is not a Wage Determination: The General Schedule (GS) equivalent to this position is as follows: Employee Class: Administrative Support and Clerical Occupations - GS-5 with a Monetary Wage - Fringe Benefits of $15.91 per hour. QUESTIONS AND INQUIRIES CONCERNING THIS RFQ : All questions and inquiries concerning any aspect of this announcement must be submitted in writing to carrie.e.fairbanks.civ@mail.mil for clarification sufficiently in advance of the deadline of receipt of the quote to get an answer in time to meet that deadline. Telephone requests will not be honored. The Government's response to inquires will be made in writing. Any resulting additions, deletions or changes to the RFQ will be made by issuances of a formal amendment. Quoters are instructed specifically to contact only the issuing contract office in connection with any aspect of this requirement prior to contract award. PLEASE NOTE : A quotation does not constitute an award; therefore, an obligation to your company is not official until you receive a purchase order for this service. Quotes are due no later than Monday, March 30, 2015 by 10:00 am (Eastern). Pending Q&A document that will posted under Amendment 2. Closeing date will be extended. Quotes are due no later Thursday, April 2, 2015 by 10:00 am (Eastern). The Pending Q&A document was no longer required. The quantity of estimated bins for Site 1 has changed from 15 - 30 bins to 20 - 25 bins for the base and all option years. The bins for hard drives have been removed and are no longer applicable to this requirement. These changes resulted in a revised Statement of work (SOW) and Schedule of Items table. Please use the updated table for submission of quotes. If you have already submitted your quote, please submit a revised schedule of items table before the closing date. If a revised table quote is no received, it will not be considered valid due to the change in quantity for the above line items.
