RFP Section B-E SF-1449 Attachment D-4 Attachment D-3 Attachment D-2 Attachment D-1 Dear Prospective Offeror: Enclosed is Request for Proposal (RFP) No.: 245-S15-0005. The Navajo Area Indian Health Service (NAIHS), Gallup Indian Medical Center is intending to establish an Indefinite Quantity Indefinite Delivery type contract using Firm Fixed Price with a responsible and responsive contractor for the provision of medical coding services for the Gallup Service Unit medical facilities: 1) Gallup Indian Medical Center and 2) Tohatchi Health Care Center. The attached document submission checklist and blank copies of SF-1449, Sections B and E must be completed as part of your RFP proposal package. Your proposal shall be prepared in accordance with E-1 "Instructions to Offerors-Commercial Items". This section stipulates the submission of all required documents, certifications, qualifications, etc., to determine your responsibility. The solicitation is 100% TOTAL SMALL BUSINESS SET-ASIDE. The official closing date for this RFP is at 2:00pm (DST) on April 10, 2015.
