- Statement of Work - Attachment 1 Technical & Price Evaluation Factors This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12, in conjunction with FAR Subpart 13 and supplemented with additional information included in this notice. This announcement constitutes the only solicitation and written quotations are being requested. OFFERORS ARE ADVISED AN AWARD MAY BE MADE WITHOUT DISCUSSION OR ANY CONTACT CONCERNING THE OFFERS RECEIVED. Offerors should not assume they will be contacted, or afforded the opportunity to qualify, discuss, or revise their proposals. However, the Government may contact offerors for the purpose of clarifying aspects of their proposal. The Government reserves the right to reject any or all offers prior to award; to negotiate with any or all offerors; to award the contract to other than the lowest total price; and to award to the offeror submitting the offer determined by the Government to be the most advantageous to the Government. Therefore, the initial offer should contain the offeror's best terms from a technical and price standpoint. This is a total small business set-aside. The North American Classification System (NAICS) code is 624110 with a small business size standard of $11,000,000.00 for this requirement. Offers are due on Wednesday, March 25, 2015, by 2:00 PM Eastern Time at the Centers for Disease Control and Prevention, Procurement and Grants Office, Office of Acquisition Service, MS-K71; Attn: Philippa D. Butts, 2920 Brandywine Road, Atlanta, GA 30341. Offers will be accepted by email at: PButts@cdc.gov. The Offeror shall cite RFQ Number 2015-N-16899 in the email subject line. The Offeror must have an active registration in the System For Award Management (SAM) database (www.sam.gov) prior to award of the purchase order in accordance with FAR Clause 52.204-7. The Centers for Disease Control and Prevention requires the following items: CLIN 0001 (Base Year) CHILD CARE SUBSIDY TUITION PROGRAM ADMINISTRATOR. The period of performance shall commence on date of award and continue for one year. See attached Statement of Work. Quantity 1 each for 12 months. CLIN 0002 (Option Year One) CHILD CARE SUBSIDY TUITION PROGRAM ADMINISTRATOR. The period of performance shall be for one year. See attached Statement of Work. Quantity 1 each for 12 months. CLIN 0003 (Option Year Two) CHILD CARE SUBSIDY TUITION PROGRAM ADMINISTRATOR. The period of performance shall be for one year. See attached Statement of Work. Quantity 1 each for 12 months. CLIN 0004 (Option Year Three) CHILD CARE SUBSIDY TUITION PROGRAM ADMINISTRATOR. The period of performance shall be for one year. See attached Statement of Work. Quantity 1 each for 12 months. CLIN 0005 (Option Year Four) CHILD CARE SUBSIDY TUITION PROGRAM ADMINISTRATOR. The period of performance shall be for one year. See attached Statement of Work. Quantity 1 each for 12 months.
