Description
Amendment One Amendment No. One is hereby issued to Solicitation No. 15-247-SOL-00045 for the following: 1.Extend the solicitation closing date to 3/23/15 at 12:00 p.m., Phoenix, Arizona Time. 2.Provide Pre-Proposal Inquiries (PPI), 10 March 2015, as follows: PPI-001: B-1. INTRODUCTION, "In order to achieve this mission, IHS requires the demolition and disposal of two modular units located at the Phoenix Indian Medical Center." Is this scope contained in this statement to be included in the project? Answer: This is a typo PPI-002: C-11. SUBCONTRACTING, a. "None of the services furnished under the Statement of Work shall be subcontracted or transferred to another Contractor or organization without the specific prior written approval of the Contracting Officer." Can a SB, 8(a) Native Owned Prime Contractor bid this project with a lower tier Roofing Contractor? It is referenced under section C-11.e. that we shall employ specialty contractors. Answer: yes, as long as the Prime Contractor complies with the sub-contracting clause PPI-003: Has the existing roof underlayment or any associated roof mastic been tested for asbestos containing materials? If not, will this be required of the contractor or completed by the government? Answer: No tests have been completed. The contractor will be required to complete any tests required. There does not appear to be any underlayment, at least on the overhang. Please see attached photograph of the deck below the tile. Preliminary investigations indicate only accumulated dirt between the tile and sheathing. The portion of sheathing on the overhang is to tongue and groove material. (see Attachment D) PPI-004: Part 2.f.i.1.a. "Remove sheathing that is not in good condition." Shall we price this as a SF Unit Cost or do you have a standard square footage not-to-exceed price within the scope of the project? Answer: Yes standard square footage, allow for 10% damaged sheathing PPI-005: C-19. QUALIFICATIONS, "The prime contractor must have a minimum of three (3) years of experience with similar project." Can the experience of the lower tier Subcontractor be used for the proposal if they are identified in the submitted proposal? Answer: Yes PPI-006: H-2.(f) "The Contractor shall insert the substance of this clause in each subcontract involving the use of hazardous materials or operations." Hazardous materials are not referenced. Is this in regards to the VOC Compliance Plan requirements typically utilized at PIMC? Answer: This is an overall requirement, you must comply with section C-15 Post award submittals PPI-007: Are interior dust control measures to be utilized on this project? Answer: No PPI-008: Will this building be occupied during construction? Answer: Yes. Part 2 e states that temporary weather proofing is required. PPI-009: Will temporary site fencing be required for this project? Answer: it is recommended, but not required PPI-010: What relevance will 352.270-3 Indian Preference Program JAN 2006 have on the selection process for this solicitation? Answer: No relevance PPI-011: Please confirm that the Tax Exemption referenced is for permanently installed materials only.
