- *** See Attachments for Complete Details and Spreadsheets*** The Uniformed Services University of the Health Sciences (USU) is the Nation's federal health sciences university and is committed to excellence in military medicine and public health during peace and war. We provide the Nation with health professionals dedicated to career service in the Department of Defense and the United States Public Health Service and with scientists who serve the common good. We serve the uniformed services and the Nation as an outstanding academic health sciences center with a worldwide perspective for education, research, service, and consultation; we are unique in relating these activities to military medicine, disaster medicine, and military medical readiness. USU is located in Bethesda, MD on the grounds of the Naval Support Activity Bethesda. Additional information regarding USU and its mission can be found on our website: www.usuhs.edu. USU has an ongoing requirement for mice and rats to be used in support of research studies at the Uniformed Services University of the Health Sciences (USU), to include Armed Forces Radiobiology Research Institute (AFRRI). The resultant multiple awards will be a firm-fixed priced (FFP) blanket purchase agreement (BPA), with a performance period of five (5) years. This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes (RFQ) for commercial items in accordance with the procedures of FAR part 12 - Acquisition of Commercial Items and FAR part 13 - Simplified Acquisition Procedures utilizing the FAR subpart - 13.5 Test Program for Certain Commercial Items. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular (FAC) 05-78 (December 26, 2014) and Defense Federal Acquisition Regulation Supplement (DFARS) (January 29, 2015). In order to be eligible for award, offerors must be registered in www.SAM.gov, with completed representations and certifications by close date of solicitation. Historical Data During Fiscal Year (FY) 2014, (October 1, 2013 until September 30, 2014) 53 calls were placed against the previous mice BPA with Harlan Laboratories (HT9404-12-A-CV39) for a total cost of $126,892.32. In addition, three mice contracts (HU0001-14-M-V308, HU0001-14-M-V312, and HU0001-14-M-V364) were awarded to Jackson Laboratory during this same period for a total value of $45,408.00. The spreadsheet below provides the quantity of rodents purchased by strain during the past two fiscal years (October 1 - September 30). These requirements are estimates only and are not guaranteed. It is expected that the first year requirements will be lower in comparison to the remaining four years due to maintenance work at AFRRI. Offerors are required to provide pricing for the strains of rodents listed below by completing the highlighted cells in Attachment 1. Future orders will not be limited to the strains listed in this spreadsheet. If other strains are available for inclusion in the BPA, offerors are encouraged to include them with the submitted quote. The Government reserves the right to add BPA holders at any time during the life of the agreement. Evaluation factors are listed in order of importance, with most important listed first. The government will conduct a trade-off between price and non-price factors. The Government is not required to award to the lowest priced offeror or the highest technically rated offeror. The government reserves the right to award without discussions. Evaluation Factors (Listed in order of importance): 1.Corporate Experience: Offerors are required to provide a narrative of their corporate experience in supplying mice and rats for research purposes. Experience in providing the specific strains include in the estimated requirements spreadsheet may be considered more desirable by the Government. Maximum Number of Pages: 3 pages 2.Technical Experience: Offerors must submit literature for the quoted animals and provide a description of the housing for the animal and the transportation method. At a minimum, the literature must demonstrate that the animals quoted meet the minimum specifications. Offerors must also provide literature on their breeding program and how they maintain adequate levels of stock for the mice and rat strains identified. Offerors must also provide delivery lead time after receipt of order. As part of evaluating corporate experience, USU reserves the right to visit the offerors' facilities prior to award. Maximum Number of Pages: 7 pages 3.Past Performance: The Government shall evaluate the offeror's present and past performance on similar tasks within the past three years. Offerors must provide two (2) customer references for similar work performed. The Government reserves the right to select which reference(s) to contact. The following must be provided for each listed reference: •Name of client and address •Name of current point of contact including telephone number, email address and title •Contract number •Contract value •Contract period of performance •A clear description of the supplies/services provided USU may request additional information at any time. Offerors will have an opportunity to address any adverse past performance history. Offerors who do not have any past performance history will not be rated favorably or unfavorably, but will instead receive a Neutral/Unknown rating for past performance. Offerors must clearly state in their quote if they do not have at least two (2) past performance references. Failure to provide this information may render an offeror's quote non-responsive and may not be considered for award. A strong history of relevant past performance may be more advantageous to the Government than a less relevant record of favorable performance. The Government reserves the right to use data provided by the offeror and data provided by alternative sources in conducting its assessment of past performance history. Maximum Number of Pages: 2 pages 4.Price: Offerors are required to complete the highlighted cells in the pricing Excel spreadsheet provided as Attachment 1. Prices will be evaluated by calculating the total cost of all five years of pricing provided in the spreadsheet. For evaluation purposes, the cost of shipping will be evaluated using 10 deliveries per year. Offerors must provide a fixed price unit cost and total cost for the full duration of the BPA. Offerors are requested to include prices for other available strains of mice and rats that can be incorporated in the BPA. Offerors must include all applicable costs (Example: Animals, Containers, Shipping) in their quote. Prices can be reevaluated annually on the anniversary of the date of award and can be adjusted to a mutually agreeable price. If the presented price is determined not to be fair and reasonable, the Government reserves the right to not utilize the BPA. The quantities listed are estimates only and are based on the traditional five percent increase in rodent purchases. See attached list for applicable provisions and clauses for this requirement. Offerors intending to respond to this request must provide notification to sherri.ori@usuhs.edu by 7:00 PM Eastern on February 27th. Questions/clarifications regarding this solicitation must be submitted via email to sherri.ori@usuhs.edu by March 4th. Any question received after March 4, 2015 will not be answered. Questions will not be answered over the phone or submitted by any other means. All questions will be answered in an amendment to the solicitation posted on or around March 10th. Quote packages are due by 10:00 PM Eastern on March 18, 2015. Quotes must be submitted via email to sherri.ori@usuhs.edu. Quotes will not be accepted by any other means. Late quotes will not be considered. Quote packages should include a cover sheet that provides the following information: •Company Name •Point of Contact, to include email and phone number •DUNS Number •Statement that includes acknowledgement of latest amendment number BPA STATEMENT OF WORK (SOW) Inspection and Acceptance The Contractor shall only tender for acceptance those rodents that conform to the requirements of this contract. The Government reserves the right to inspect or test any mice or rats that have been tendered for acceptance. Vendors must make their animals and all premises, facilities, vehicles, equipment and records that support animal care available during business hours and at other times mutually agreeable to the vendor and the Government. Contractor must make the requested premise, facility, vehicle, equipment or records available within 48 business hours upon receiving a written request from the Government. All animal husbandry, breeding, and shipping practices should be in accordance with the Animal Welfare Act P.L. 89-544 and the Guide for the Care and Use of Laboratory Animals. These animals may be sent from different barrier facilities owned and operated by the same company as long as the husbandry, health and treatment conditions mirror each other. The animals must be adventitious disease-free. All animals should be raised with restricted access and strict entry and exit policy in caging that reduces the exposure to pathogens such as in barrier condition, microisolation caging or isolators. They must be free of pathological lesions as assessed by gross exam. Health surveillance monitoring must be performed regularly (for e.g., quarterly) on the colonies and verified by an outside source. Health Surveillance reports must be sent with the animal shipments and must be current (for e.g., within the last 6 months) as well as accurate. All shipments must be tested negative for Pseudomonas aeruginosa by PCR or culture within one week prior to being shipment to AFRRI. Diet and Bedding Monitoring: Standard laboratory animal diet should be used, Teklad preferred. Diet should be manufactured under ISO 9000 procedures with careful ingredient sourcing and monitoring. High quality bedding and bedding enrichment products should be manufactured following appropriate quality standards and screened periodically for contaminants, with certification available. Vendors must provide notification by email to VSD and Contracting (CTR) when changes are made to the care or treatment of animals. This includes any changes to the bedding or the diet of the animals. Delivery of animals should be in climate-controlled vehicles. Animals should be packed, shipped and delivered in the same day to reduce shipping stress; a trip of less than 5 hours is preferred. Temperature and humidity should be monitored in real time. Deliveries must be made in accordance with requested delivery schedule. Vehicles should be subject to stringent cleaning and testing before and after each use to ensure that animals are not exposed to pathogens during transport. Vehicles should be ISO 9001:2000 certified. Vehicles should be dedicated to the shipment of research animals and supplies. Animals should be shipped with food/water supplies appropriate for the length of shipment in shipping containers that meet all standard animal transportation practices. If an awardee no longer meets the technical standards required above and is determined to be unacceptable, the contractor is no longer eligible to receive orders under the BPA. Before being reconsidered for new orders, all deficiencies must be addressed in a written plan of action and submitted in writing to VSD and CTR. If VSD and CTR agree the plan is acceptable and will protect the Government against future occurrences, the vendor's participation in the BPA will be restored. Damages: The Government may require repair or replacement of nonconforming animals at no increase in contract price. If repair/replacement or reperformance will not correct the defects or is not possible, the Government may seek an equitable price reduction or adequate consideration for acceptance of nonconforming supplies or services. The Government must exercise its post-acceptance rights- (1)Within a reasonable time after the defect was discovered or should have been discovered; and (2)Before any substantial change occurs in the condition of the item, unless the change is due to the defect in the item. If a vendor knowingly fails to follow the requirements provided in the Statement of Work or withholds critical health information concerning the vendors supply colonies that causes damage to the ordering investigator or other existing AFRRI rodent colonies, the contractor may, in place of actual damages, pay the Government liquidated damages. The value of the liquidated damages will be calculated based on the cost to purchase replacement animals from another provider, to include shipping and handling, and any grant funds lost due to the nonconforming animals. Funding Funding will be provided at the time of issuance for each call. Department will submit a bulk fund deposit request to Financial Management prior to placing any call. Ordering Instructions After receiving a new requirement for an approved protocol, VSD at AFRRI will email the request to BPA awardees using the Mice and Rats Blanket Purchase Agreement New Requirement Order Form (Attachment 2). Vendors are to complete the unit price for each year of the requirement. Vendors have the option of providing a more competitive price for individual order requests, but the price must not go above the prices listedin the BPAaward.. Quantities provided are estimates only and vendors must honor the unit price quoted regardless of the actual quantity required. The vendor providing the lowest overall cost for the individual order will be selected for the duration of the requirement or the performance end date of the BPA, whichever occurs first. Once a vendor has been selected, a copy of the order form shall be provided to CTR by VSD. Existing protocols and new protocols that require the same strain or colony for comparison purposes will continue to use the approved vendor. All authorized calls made against this BPA will be paid for with Government Purchase Cards by authorized cardholders. The preferred method for ordering under this contract shall be either via phone or electronic correspondence (email). The individuals listed below have been identified as authorized callers against the BPA: The authorized callers listed below are the only persons authorized to make calls against this BPA. This may be amended to include new people or exclude current authorized callers. The addition or removal of authorized callers must be made by a modification to the BPA. The contractor shall not accept any orders against this BPA from any person not identified below, excluding a Contracting Officer at USU. Authorized Callers: To Be Determined Extent of Obligation The Government is obligated only to the extent of authorized calls made under the BPA. At no time shall this BPA be considered a requirements contract. USU reserves the right to utilize any source for their mouse requirements. Payment Instructions Payment will be made once a call is issued by the authorized callers. The selected contractor will request Government Purchase Card (GPC) information when the call is placed, but will not charge the GPC until the order has shipped. Reporting Contractor will be required to submit a monthly activity report to the contract specialist. The activity report is due by the fifth business day of the month and must list the activity from the previous month. The report must be submit via email to the email address identified. The report, at a minimum, must contain the following information: •Date of each call •Qty and strain of mice and rats ordered •Name of caller •Amount of each call •Delivery date
