- Performance-Based Work Statement SOURCES SOUGHT synopsis announcement to assist the 673d Contracting Squadron, Joint Base Elmendorf-Richardson (JBER), Alaska, in performing market research to determine interest and capability. This is not a request for proposal (RFP) and responses will not be accepted by the Government to form a binding contract. This is not a guarantee of a future RFP or an award and should not be construed as a commitment of any kind by the Government. Any information provided to the Government in response to this document will become 673d Contracting Squadron property and will not be returned. The Government will not reimburse contractors for any cost associated with responding to this request. JBER, AK is seeking interested sources capable of providing a System Engineer IV and Program Manager I to inspect, report, service, and provide preventive maintenance inspections and repairs necessary to keep the 673d Medical Group Hospital Picture Archiving & Communication Systems (PACS) equipment functioning in a safe and reliable condition. The contract is tentatively planned to run for a base period of one year with four option years beginning 1 October 2015. The applicable North American Classification System (NAICS) code is 541513 with an SBA business size of $25,000,000. Please see attached Draft Performance Work Statement for further information. Please note there are three options for additional services (Appendices 3, 4, and 5) that MAY be required. Interested and qualified sources are requested to provide: (1) company name, point of contact, e-mail address, telephone and fax numbers, type and business size under the applicable NAICS code; (2) a list of the company's most recent and relevant (within the last three years) DoD or commercial contracts to include names, points of contact, phone numbers, value and type of contract, and brief description; and (3) information demonstrating the company's ability to perform these services. Respondents are asked to limit submissions to five pages or less. Any information provided is voluntary. All eligible firms responding to this Sources Sought must ensure their response is complete and sufficiently detailed to allow the Government to determine the firm's qualifications to perform this requirement.
