Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-78B/79 issued and effective December 26, 2014. This solicitation VA259-15-Q-0259 is issued as a request for quote (RFQ). The North American Industry Classification System Code (NAICS) for this procurement is 339113 (Surgical Appliance and Supplies Manufacturing) using a SBA designated Small Business Size Standard of 500 employees. This RFQ is being issued as a small business set-aside under NAICS Code 339113. Department of Veterans Affairs, VISN 19, Eastern Colorado Health Care System (ECHCS) Denver VA Hospital is soliciting offers from qualified and capable sources. Respondents to this combined synopsis/solicitation must fully demonstrate their capability by supplying detailed quote information along with any other documents necessary to support the requirements below. Solicitation and evaluation of offers are to be IAW FAR 52.211-6: Brand Name or Equal. C.Trolley Insert Slot a)Allows multiple users at a time by adding and removing trolleys as needed depending on the patient therapy work load. D.Lanyard a)Adjustable cam release allows therapist to adjust tension for patient and ceiling height. b)Single line system connecting the patient to the trolley. c)Back up adjustable safety cam provided and serves as an extra safety precaution. d)360 degree swivel at top and bottom allows unfettered movement. e)Height adjustment feature at the patient level. 6.Shop Drawings A.Provide drawings in.CAD format for the mounting and installation of the system. Drawings and structural calculations must be provided and approved by VA within 21 calendar days post award date. 7.Training A.Provide training literature, user manuals, and technical support contact information (if not available for download on vendor's website) B.On-site end user training not required 8.Past Performance A.Vendor must provide documentation of government facilities where equipment is already in use. B.A total three references and points of contact must be provided. 9.General Requirements A.All work to be coordinated with VA upon award of contract B.Delivery Address: a)VA Medical Center 13611 E Colfax Ave Aurora, CO 80041 C.Include Lift Gate on truck D.Ensure off-loading of truck by contractor, not VA E.Ensure one shipment of product. Multiple deliveries/multiple dates are not authorized unless otherwise approved by VA F.Vendor is responsible for any special handling or storage requirements. G.Estimated delivery date is 4/6/2015 H.72 hour advance notification to Primary or Alternate POC required prior to delivery: a)Delivery POC information will be disclosed upon award. 10.Acceptance A.Contractor's equipment once delivered will be inspected by the VA for completeness of order and for any damage/defect to the equipment before full payment will be awarded. All items must be covered by the manufacturer's warranty. SAM Requirement: This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines.
