The Department of Veterans Affairs (VA) is performing market research to assess the commercial marketplace for Ambulance vehicle transportation to Veteran Affairs beneficiaries receiving care at the Wilkes-Barre VA Medical Center, 1111 East End Blvd., Wilkes-Barre, PA 18711, and associated outpatient clinics: i.Allentown, Pennsylvania Community-Based Outpatient Clinic 1.3110 Hamilton Boulevard 2.Allentown, PA 18103 ii.Bangor Clinic 1.701 Slate Belt Boulevard 2.Bangor, PA 18013-9341 iii.Berwick Community-Based Outpatient Clinic 1.Alley Medical Center, 301 West Third Street 2.Berwick, PA 18603 iv.Tobyhanna Community-Based Outpatient Clinic 1.Building 220 2.Tobyhanna, PA 18466 v.Williamsport Community-Based Outpatient Clinic 1.1705 Warren Avenue, Werner Building - 3rd Floor (Suite 304) 2.Williamsport, PA 17701 There is no solicitation document as this time. The NAICS is 621910 with Small Business size of $15 million. The Department of Veterans Affairs is contemplating indefinite delivery, indefinite quantity (IDIQ) contracts for up to five years in duration for ambulance services. A DRAFT Performance Work Statement (PWS) is attached. Please read through the DRAFT PWS before responding to this notice. If your company provides ambulance services to either all or some of the areas identified, please provide the information requested below: 1.Company Name 2.Company Duns & Bradstreet Number 3.Socio-economic Status (e.g. large business, small business, service-disabled veteran owned small business, etc.) 4.Point of Contact with Name, Email Address, and Telephone Number 5.What service areas does the company currently operate? Please identify the county(ies). Providing a map of coverage areas would be helpful, but is not mandatory. 6.Are there any exclusivity agreements applicable to service areas? If so, please provide general details. 7.How many ambulance vehicles does the company have? Please describe the average age of the fleet.
