- RFQ Scope of Work for Optometry Services at MTC/Denison Job Corps Center THIS IS A SUBCONTRACTING OPPORTUNITY with Management & Training Corporation (MTC), operator of the Denison Job Corps Center located at 10 Opportunity Drive, Denison, IA 51442. MTC is requesting bids from licenced Optometrists to provide Optometry services. This is an off center contract to be performed at awarded contractors place of business. This is a 6 month contract with two 3 month option periods beginning April 1, 2015. Denison Job Corps Center is a federally funded residential, educational and vocational training facility for young adults 16-24. Services performed are included in the attached RFQ/Scope of Work. All bids must be hand delivered or returned by mail, electronically or fax; and received no later than 3:00 p.m. March 11, 2015. Your response must be mailed or delivered to: Linda Boyd MTC/Denison Job Corps Center 10 Opportunity Drive Denison, IA 51442 MTC reserves the right to reject any or all bids or waive any formality connected with the bids. MTC will be the sole judge of which bid is the best value and its determination and finding on such decision will be final.
