Description
THIS SOURCES SOUGHT NOTICE IS NOT A REQUEST FOR PROPOSAL (RFP). NO SOLICITATION IS BEING ISSUED AT THIS TIME. THIS NOTICE SHALL NOT BE CONSTRUED AS A COMMITMENT BY THE GOVERNMENT TO ISSUE A SOLICITATION OR ULTIMATELY AWARD A CONTRACT, NOR DOES IT RESTRICT THE GOVERNMENT TO A PARTICULAR ACQUISITION APPROACH. ANY INFORMATION PROVIDED TO THE GOVERNMENT AS A RESULT OF THIS SOURCES SOUGHT NOTICE IS VOLUNTARY. RESPONSES WILL NOT BE RETURNED. NO ENTITLEMENT TO PAYMENT OF DIRECT OR INDIRECT COSTS OR CHARGES BY THE GOVERNMENT WILL ARISE AS A RESULT OF CONTRACTOR SUBMISSION OF RESPONSES OR THE GOVERNMENT'S USE OF SUCH STATEMENT OF WORK/DESCRIPTION OF WORK GENERAL INTENTION Contractor shall furnish all labor, materials, parts, equipment, tools, transportation, and calibration to provide Carl's Tables to the Michael E DeBakey VA Medical Center, 2002 Holcombe Blvd. Houston TX 77024. The Contractor shall provide all necessary training to the designated VA employees on how to use the product after the delivery and setup are complete. The contract price shall be firm-fixed, all inclusive not limited to specified products/equipment, installation, setup/configuration, and training on how to use the installed products/equipment. All equipment shall be brand new; no refurbished or used equipment is acceptable. CONTRACT REQUIREMENTS 1.MEDVAMC would like to acquire two new Carl's Tables for the new Breast Care Program with the following specifications: "2 electrically controlled shelves with height and tilt adjustments, side shelf, back privacy panel, two height-adjustable legs, cpu holder, footrest, 2 background lights, 2 LED task lights, 2 control panels, powered USB hub, Air ventilations system, 2 monitor brackets and cable management troughs, 3 power strips with 14 outlets, and a 10' power cord with cable management sleeves. "100 programmable preset positions "Adaptive lighting system with automatic sensors to adjust ambient lights "3 flat panel monitor arms per table - vertical range of 16" and rotate 360 degrees. Arm must support monitors up to 42 lbs. "Table must come with state of the are ergonomic chair 2.End User Instruction and Training. Contractor shall provide on-site training to designated VA employees on how to use all the aforementioned system. Contractor shall coordinate with the Contracting Officer's Technical Representative for the schedule of this training. Training shall consist of proper use of the system, management of the system and maintenance of equipment. 3.Service. Contractor will provide one year or service for the equipment. Service contract will also include option years for this service to be extended for up to four (4) additional years. Service will meet the following minimum requirements: a.24 hour phone support Monday through Friday excluding government holidays. 4.User's Operating and Maintenance Manual. Contractor shall provide a complete set of User's Operating and Maintenance Manual at no charge to the Government. 5.Delivery Schedule. Contractor shall finish all works of installation, configuration, and setup within 30 days after receiving Notice to proceed from the Contracting Officer. INSTRUCTIONS FOR SUBMISSION After review of the above information, interested parties may submit a response in an electronic format only via email to albert.thibodeauxjr@va.gov not later than March 6th, 2015 3:00 pm CST. Electronic files should be in Microsoft Word or PDF format. ALL INFORMATION SHOULD BE UNCLASSIFIED MATERIAL ONLY. Respondents should include as part of their submission: a.A synopsis of the company's capabilities to provide qualified contracting support personnel on company letterhead. b.Company point of contact telephone number and email address. c.Socioeconomic status Any company proprietary information must be marked as such. The total number of pages for the response is limited to 10 one-sided 8 ½ x 11 pages with one inch margins and font no smaller than 12 point.
