Solicitation Number: VA247-15-Q-0338 Notice Type: Combined Synopsis/Solicitation Synopsis: This is a COMBINED SYNOPSIS/SOLICITATION for commercial services prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed under FAR 13.1, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA247-15-Q-0338 and is issued as a Request For Quotation (RFQ) (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-52, November 2, 2011. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The RHJVA Medical Center will provide equipment for fit testing of employees: TSI Portacount Pro+ fit testes (2) with associated attachments/equipment, probes and probe equipment, one portable HP Deskjet 1010 printer, USB cable, respirators. Contractor will travel throughout the Medical Center to each service with respirator fit test cart containing all the necessary fit testing equipment and supplies. Contractor shall probe all needed N95 masks in preparation for fit testing during downtime (between employee fit tests/sessions). QUANTITATIVE FIT TEST (QNFT): A QNFT is an assessment of the adequacy of respirator fit by numerically measuring the amount of leakage into the respirator. A QNFT is required for all negative pressure full facepiece respirators that are to be used in atmospheres greater than 10 times the OSHA PEL Our standard QNFT method uses an Ambient Aerosol Condensation Nuclei Counter (TSI Portacount ). A fit factor of >500 will be considered a pass for full facepiece respirator and all Health Care Particulate Respirators receive a 100 fit factor particulate. In the event of a pandemic healthcare event/emergency or Portacount equipment malfunction, then qualitative fit test may be performed with approval from COR. In such an event, contractor shall provide the qualitative equipment. Contractor or technicians to provide services to be shall reside within the commuting area to RHJVA MC (less than50 miles radius). CONTRACTOR'S RESPONSIBILITIES: In accomplishing any work under this contract the contractor is totally responsible for his/her full compliance with any and all applicable, federal, state, and local government policies, regulations, procedures, specifications, etc., that apply to work of this nature. In addition, the contractor must obtain and keep current all licenses, OSHA training, permits, etc., that may be required by the above mentioned agencies to accomplish the work. The contractor at the beginning of each annual fit test (not initial) will have each employee fill out the required RHJ VAMC Annual Respirator Medical Surveillance worksheet. By answering five simple questioned to ensure employee has had no major changes to preclude them from being fit tested. The contractor at the conclusion of each quantitate fit test shall print and have each fitted wearer sign the Portacount fit test report which includes: (a)Date Tested; (b)Assigned Care Line/Service; (c)Name of employee tested; ID Number (last 4 of SSN) (d)Fitted respirator make, model, style, size, and filter type/number, if applicable; (e)Type of fit test performed, i.e. QNFT; (f)Pass/Fail test results (Fit Factor test results if QNFT); The contractors will use Microsoft Access rosters provided to them by Contracting Officer's Representative (COR) and turn in all, annual surveillance worksheets, Portacount fit test or PAPR training reports to the COR at the end of each fit test session/day. All PHI and PII will remain within the RHJ VA Medical Center. CONTRACTOR'S WORK HOURS - Main Hospital: All fit tests performed at the RHJ VAMC shall be between the hours of 7:00am and 5:00pm, Monday thru Friday except Holidays,. The COR in coordination with the Contractor and service chiefs will schedule weekend work (Saturday or Sunday) or testing times to accommodate personnel shift changes in early morning, mid-afternoon and mid-nights on mutually agreed upon dates and times upon receiving an initial request 2 business days prior to the requested training date. The frequency of training shall be minimum of two days per month. Contractor shall perform quantitative fit tests/training on maximum of forty eight employees each day. Contractor shall provide the names of technicians and the applicable contact information (i.e., phone numbers and emails, etc.) to the COR, that are scheduled to work at any VAMC site at least 48 hours before arriving on VAMC hospital premises. CONTRACTOR WORK HOURS - Myrtle Beach Community Based Outpatient Clinics (CBOCs): Onsite respiratory fit test and training shall be scheduled and performed between the hours of 0700am and 1600 hours, Monday thru Friday except Holidays, as needed. COTR will provide the contracted services information to the AO and Nurse Manager prior to arrival to clinic in Myrtle Beach, SC. The contractor shall be required to train and fit-test any personnel identified by the COR, involving any additional CBOC during this contract period. The frequency of training will be required at minimum of two days annually. for a maximum of forty eight employee each day. Government per diem & mileage rates shall be consider by contractor for the two travel days to the MB CBOC. RESPIRATOR FIT-TEST TRAINING: Contractor shall provide training to two major categories of respirator users at this facility (Involuntary/Mandatory and Voluntary/Not Required-Appendix D), which as a minimum must include: (a)Instruction on Uses & Limitations (b) Inspection (c) Donning & Doffing (d)User Seal Check (e)Maintenance & Storage REFERENCES: Contractor shall furnish at least two (2) references of past or current customers of similar or larger size and complexity to that of RHJ VAMC Charleston. The references are preferred to be within the healthcare industry. CONTRACTOR ASSESSMENT OF TEST SUBJECT: Contractor shall ensure an assessment of comfort and fit of the respirator is performed on each test subject and that the test subject is allowed adequate time to determine the comfort of the respirator using the following points of view: (A mirror will be provided on fit testing cart) (a)Position of respirator on the nose; (b)Room for eye protection; (c)Room to talk; (d)Position of the respirator on the face and cheeks; (e)Fit across the bridge of the nose; (f)Respirator of proper size to span distance from nose to chin; (g)Tendency of respirator to slip; CONTRACTOR FIT-TEST ASSESSMENT FOR TEST SUBJECT:Contractor shall use an OSHA-Accepted Fit Test Protocol (i.e. QNFT), per OSHA 1910.134 for each test subject fitted and tested. Contractor shall ensure the test subject conducts a user seal check, either the negative and positive pressure seal check as described in OSHA Standard Appendix B-1 or those recommended by the respirator manufacturer which provides equivalent protection to the procedures in Appendix B-1. The Contractor shall discontinue or not fit test a test subject, if any of the following are detected: (a)Hair growth, such as stubble beard growth, beard, mustache or sideburns between the skin and respirator face-piece that prevents or interferes with respirator face seal; (b)Any apparel which interferes with a satisfactory fit; (c)Test subject exhibits difficulty in breathing during the tests, she or he shall be referred to a physician or other licensed healthcare professional (i.e., Employee Health Clinic), as applicable, to determine whether the test subject can wear a respirator while performing her or his duties; (d)If the employee finds the fit of the respirator unacceptable, the test subject shall be given the opportunity to select a different respirator from the approve list of respirators identified within the statement of work, and retested; CONTRACTOR PERFORMANCE MEASURES: (a)Contractor fails to certify and provide a detailed work and training history of all trainers/training personnel that will work onsite at any VAMC Charleston facility or facilities: (b)Contractor fails to maintain and provide copies of certificates or proof of having received at least the appropriate level of respirator protection training or experience for a program administrator in accordance with the program's level of complexity. RHJ VAMC shall provide and ensure a medical evaluation to determine the employee's ability to use a respiratory before the employee is fit tested or required to use the respiratory in the workplace per OSHA 1910.134 (e). The 3M 1860 N95 Disposable Particulate respirator will be used as the primary respiratory device of choice for fitting and testing all healthcare employees, unless a fit/seal cannot be made or employees has facial hair. In this case, the employee will be trained on use and care of using 3M Air-mate PAPR. Employee will be used their own3M BE-12 hood to be used with respective workplace PAPR(s). PERIOD OF PERFORMANCE: Base Year plus 4 one-year Options MANDATORY ELECTRONIC INVOICING: Vendors can comply with the VA mandatory electronic invoicing rule by using either of the two methods below: 1. The Financial Service Center uses a third-party contractor, OB10, to transition vendors from paper to electronic invoice submission. A system that conforms to the X12 electronic data interchange (EDI) format established by the Accredited Standards Center.
