Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA262-15-Q-0118, and this solicitation is a Request for Quote (RFQ). This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The VA Greater Los Angeles Healthcare System (VAGLAHS) has a requirement for dosimetry devices and services. This RFQ is a Brand Name or Equal solicitation. All items shall be new, and not refurbished or recycled. Specific requirements are listed on the attached Schedule of Supplies and Services and Statement of Work. Offerors should thoroughly review the Schedule of Supplies and Services and Statement of Work, and be familiar with the requirements of the solicitation prior to submitting quotes in order to be fully aware of the scope of supplies and services required. Failure to do so will not relieve the successful offeror from performing in accordance with the strict intent and meaning of the specifications without additional cost to the Government. The Government shall not reimburse any cost not incorporated into the offeror's price. Respond to the requirements of this RFQ by completing the blanks in the Schedule of Supplies and Services and by including FAR 52.212-3 Offeror Representations and Certifications-Commercial Items with quotes. Both are attached to this RFQ. Also, vendors shall include local stock numbers with quotes. Email quotes to Susanne.Christen@va.gov. Please ensure that the subject line of all emails include "VA262-15-Q-0118." Quotes are due by 9a.m. EST on February 19, 2015. Those providing quotes are responsible for ensuring that quotes are received by the Government prior to the specified due date and time. Those providing quotes for EQUAL ITEMS may include descriptive literature such as illustrations or drawings to indicate that the product meets the salient physical, functional, or performance characteristics identified within the solicitation. Technical inquiries can be emailed to Susanne.Christen@va.gov. Technical inquiries will be accepted through 1 p.m. EST, on February 12, 2015. For technical inquiries, please ensure that the subject line of all emails include "VA262-15-Q-0118." Telephone requests for information or questions will NOT be accepted. No hard copies of this RFQ will be provided. CLAUSES, PROVISIONS, AND ADDITIONAL INSTRUCTIONS 1. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-60. 2. The following provisions and clauses apply to this acquisition: FAR Parts 52.202-1, 52.211-6, 52.212-1, 52.212-2, 52.212-3, 52.212-4, 52.217-6, 52.217-8, 52.219-6, 52.232-39, 52.232-40, 52.233-1, 52.233-4, 52.243-1, 52.247-34, 52.249-8 VAAR 852.203-70, 852.211-70, 852.216-70, 852.232-72, 852.233-70, 852.233-71, 852.246-70, 852.246-71, 852.270-1, 852.273-73, and 852.273-74. 3. FAR Part 52.212-1, Instructions to Offerors-Commercial and FAR Part 52.212-4 apply to this acquisition and no addenda are attached. 4. FAR Part 52.212-5 Contract Terms and Conditions Required To Implement Statutes Or Executive Orders-Commercial Items, applies to this acquisition. Additional FAR clauses cited in FAR Part 52.212-5 that are applicable to this acquisition are 52.222-50, 52.233-3, 52.233-4, 52.219-28, 52.222-3, 52.222-21, 52.222-26, 52.223-18, 52.222-13, 52.222-26, 52.232-34, 52.222-36, and 52.222-50. 5. FAR Part 52.211-6, Brand Name or Equal applies. 6. The following additional FAR clauses apply to this acquisition: 6.1 52.216-18 Ordering (Oct 1995) (a) Any supplies and services to be furnished under this contract shall be ordered by issuance of delivery orders or task orders by the individuals or activities designated in the Schedule. Such orders may be issued from the effective date of this contract through the end of the effective period. (b) All delivery orders or task orders are subject to the terms and conditions of this contract. In the event of conflict between a delivery order or task order and this contract, the contract shall control. (c) If mailed, a delivery order or task order is considered "issued" when the Government deposits the order in the mail. Orders may be issued orally, by facsimile, or by electronic commerce methods only if authorized in the Schedule. (End of Clause) 6.2 52.216-19 Order Limitations (Oct 1995) (a) Minimum order. When the Government requires supplies or services covered by this contract in an amount of less than $1.00, the Government is not obligated to purchase, nor is the Contractor obligated to furnish, those supplies or services under the contract. (b) Maximum order. The Contractor is not obligated to honor -- (1) Any order for a single item in excess of $75,000.00; (2) Any order for a combination of items in excess of $75,000.00; or (3) A series of orders from the same ordering office within 1 day that together call for quantities exceeding the limitation in subparagraph (b)(1) or (2) of this section. (c) If this is a requirements contract (i.e., includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regulation (FAR)), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitations in paragraph (b) of this section. (d) Notwithstanding paragraphs (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless that order (or orders) is returned to the ordering office within 1 day after issuance, with written notice stating the Contractor's intent not to ship the item (or items) called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source. (End of Clause) 6.3 52.216-22 Indefinite Quantity (Oct 1995) (a) This is an indefinite-quantity contract for the supplies or services specified, and effective for the period stated, in the Schedule. The quantities of supplies and services specified in the Schedule are estimates only and are not purchased by this contract. (b) Delivery or performance shall be made only as authorized by orders issued in accordance with the Ordering clause. The Contractor shall furnish to the Government, when and if ordered, the supplies or services specified in the Schedule up to and including the quantity designated in the Schedule as the "maximum." The Government shall order at least the quantity of supplies or services designated in the Schedule as the "minimum." (c) Except for any limitations on quantities in the Order Limitations clause or in the Schedule, there is no limit on the number of orders that may be issued. The Government may issue orders requiring delivery to multiple destinations or performance at multiple locations. (d) Any order issued during the effective period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The contract shall govern the Contractor's and Government's rights and obligations with respect to that order to the same extent as if the order were completed during the contract's effective period; provided, that the Contractor shall not be required to make any deliveries under this contract after August 10, 2016. (End of Clause) 6.4 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days. (End of Clause) 6.5 52.252-2 CLAUSES INCORPORATED BY REFERENCE (FEB 1998) This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. Procurement for these commercial supplies and/or services is being conducted according to FAR Part 12 Commercial Items, FAR Part 13 Simplified Acquisition Procedures, and Veteran Affairs Acquisition Regulations (VAAR) as supplemented with additional information in this notice. 8. BASIS FOR AWARD: Subject to the provisions contained herein, award shall be made to the Offeror deemed responsible in accordance with the Federal Acquisition Regulation and who conforms to the solicitation requirements; and whose proposal, judged by an overall assessment of the Offeror's qualifications specified in this solicitation, meets the technically acceptable standard for the non-price factors and provides the lowest evaluated price. Non-price factors are significantly more important than price. The Department of Veterans Affairs (VA) intends to issue a Firm Fixed-Price Indefinite Delivery Indefinite Quantity (IDIQ) Order for commercial supplies and/or services.
