Description

THIS IS A SMALL BUSINESS SOURCES SOUGHT NOTICE REPRESENTING A MARKET SERUVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS. The Department of Veterans Affairs is conducting a Sources Sought to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are Service-Disabled Veterans-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or Small Disadvantaged Business concerns; and (3) their size classification relative to the North American Industry Classification System (NAICS) Code for the proposed acquisition. Your responses to the information requested will assist the organization that is not considered small business under the applicable NAICS Code should not submit a response to this notice. Small businesses concerns shall be capable of providing the necessary equipment and personnel to furnish service in the volume required for all the items under this contract. Contractor shall meet all requirements of Federal, State or City codes regarding operations of this type of service. The NAICS Code is 339113 (Size Standard: 500 employees) DESCRIPTION OF THE REQUIREMENT: 1.Title of Project: Sleep Room Beds to include Bed Base, Mattress, and Protective cover. 2.Scope of Work: The Vendor/Contractor shall provide all resources necessary to accomplish the deliverables described in this Statement of Work (SOW), except as may otherwise be specified. The Vendor/Contractor shall provide Sleep Room Beds and ensure it will meet specifications. Installation/assembly will be done by the Vendor/Contractor in the loading dock area. 3.Background: The facility Sleep Medicine Department requires Sleep Room Beds for the Atlanta VA Medical Center sleep Medicine Department area. These are adjustable and comfortable beds for the patients to sleep in while they are having a sleep study. Additionally, these beds will contribute to less clinical or to a less institutionalized sleeping environment which is the recommended national standards of the American Academy of Sleep Medicine (AASM). No remanufactured or "Gray Market" items. All equipment must be covered by the manufacturer's warranty. The patient beds shall meet the following specifications: a.Bed Base -Queen size - Ergo Premier Base -Base width - 58-60 inches -Base length - 79 inches -Bottom of base to floor - minimum 4.5 inches -Sleeping surface to floor - 25-30 inches -Full head and foot articulation in 30 seconds and bed can be lowered during power outage or remote failure. -Motorized operation -Motor - 120 volts, 60 Hz, at minimum UL listed -Hospital Grad Plug -Infinite Adjustable Positions -Manual and Wireless Controls -Minimum weight capacity - 650 lbs. -Base support by legs or locking casters b.Mattress -Queen size - Contour Rhapsody Luxe or equivalent c.Protective Mattress Cover(s) -Removable, washable cover -Anti-microbial treatment d.Delivery and Set-up 4.Type of Contract: This is a firm-fixed-price contract. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1)Potential Offerors having the skills, experience, professional qualifications, and capabilities necessary to perform the described requirement are invited to provide a capability statement via e-mail. The Capability Statement shall not exceed three (3) pages and must include the following: 1) Company name and point of contact information (address, telephone, email, web-site, etc.); 2) DUNS number and CAGE code; 3) Small Business Size/Certification (to include 8(a), HUB Zone, SDVOSB, etc.); and 4) Familiarity and product knowledge. 2)Responses are due no later than 2:00 PM EST, Thursday, February 12, 2015, and shall be electronically submitted to takya.smith@va.gov. All responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. Responses are strictly voluntary; no reimbursement will be made for any costs associated with providing information in response to this synopsis or any follow-up information requests. NO CAPABILITY STATEMENTS MAY BE SUBMITTED AFTER FEBRUARY 12, 2015. AT THIS TIME NO SOLICITATION EXISTS. DO NOT REQUEST A COPY OF THE SOLICITATION. A FUTURE NOTICE WILL BE POSTED WHEN THE SOLICITATION WILL BE POSTED. 3)Disclaimer and Important Notes: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).
