Description
STATEMENT OF WORK (SOW) as of 15Jan2015 1. Contract Title. Replace the Dental Vacuum Collector 2. Background. The Dental Vacuum System collection Tank is deteriorating and starting to leak and the system struggles to maintain vacuum pressure. This is required to for support for the Dental Services provided to the veterans. 3. Scope. Replace the separator tank (includes the discharge pump). 4. Specific Tasks. Isolate Dental Vacuum system Remove existing Wet Separator Install new 24" Wet Separator Install level switches Remove existing discharge pump Install new discharge pump Leak check Separator and pump system. Un-isolate Dental Vacuum System and perform operational retest. 5. Performance Monitoring (if applicable). NA 6. Security Requirements. The contractor employees shall not have access to VA sensitive or computer information and will not require routine access to VA Facilities. The contractor employees shall require intermittent access only and will be escorted by VA employees while at VA Facilities. No background investigation is required 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). NA 8. Other Pertinent Information or Special Considerations. a. Identification of Possible Follow-on Work. NA b. Identification of Potential Conflicts of Interest (COI). NA e. Inspection and Acceptance Criteria. Contractor will be responsible for ensuring that the Separator is leak checked and that the pump/level switches operate correctly. 9. Risk Control Contractor will provide and utilize all personal protective equipment. 10. Place of Performance. Work will be performed at the Seattle VAMC campus. 11. Period of Performance. A start date of 28 Feb15 would be acceptable and it would be completed within a weekend time frame. 12. Delivery Schedule. NA
