OF 347 Task Order Form AGAR Clauses Exhibit 2: Sample Survey Field Book Entry Exhibit I: Forest Service Handbook Chapter 30: Engineering Survey Blanket Purchase Agreement Form (to be completed and returned by vendor to the contracting office) WHAT TO SUBMIT TO THE CONTRACTING OFFICE: • Page 1 of the attached sample Blanket Purchase Agreement form with all of your company's information filled out. • Page 9-10, Representations and Certifications • Page 10-11 of the attached sample Blanket Purchase Agreement with the signature of a person capable of obligating your company when task orders are issued. • Pages 14-15 with task hourly rates (Only quote prices for the Zones where you would want to work.) Also submit technical information described on pages 12-13 for evaluation purposes.
