Description

SOURCES SOUGHT: The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 18), Southern Arizona VA Health Care System (SAVAHCS), Tucson, AZ, is currently seeking information from any company that can provide the following service/s: The SAVAHCS has a need for 0.6 FTEE Urologist Locum Tenen. The Urologist Locum Tenen shall provide male and female Urology services for the Surgical Specialty Clinic. The Urology Locum Tenen shall provide surgical treatment for genitourinary injuries and diseases including major and minor procedures, as well as inpatient and outpatient consultations. Services shall be required three (3) days per week (Monday, Tuesday and Wednesday) to include but not limited to patient examinations, in clinic procedures in the Urology lab and clinic, in addition to education and supervision of procedure training for trainees. All medical record documentation shall be done in the Computer Patient Record System (CPRS). Patients include those with all conditions and diseases relating to genitourinary specialty. The Urologist shall be required to be credentialed and privileged at the Southern Arizona VA Health Care System, and abide by the rules and bylaws of the medical staff. Rotation of evening or on-call services may be required. Definitions: The terms listed below have the following meanings: a.Contractor - in this document, Contractor refers to the company awarded this task order and all of the company's employees/subcontractors who perform services under this contract. b.0.6 FTEE - Equals the number of hours one Part Time Equivalent Employee working three (3) days per week is expected to work each year, which is 1248 hours per year or 24 hours every week for 52 weeks. This number is not inclusive of Federal Holidays. The Southern Arizona VA Health Care System (SAVAHCS) requires one (1).6 FTEE urologist Locum Tenen. Service Requirements: Urologist Locum tenen shall provide contract services three (3) days per week, Monday, Tuesday and Wednesday for the term of the agreement. The Urologist Locum Tenen shall generally perform inpatient consults and rounding 3 days a week and supervise Residents, Nurses and Nurse Practitioners performing clinic duties 3 - 4 times per week. Operating Room and Urology Lab procedures are scheduled daily five (5) days per week on a block schedule. Approximately 15-20 patients per day will be scheduled in the clinic and 3-5 patients per day will be scheduled in the Operating Room. Patient care shall be required for new and established veteran referrals. Such care includes but is not limited to examination, general history, evaluation, prescribing medications from the VA formulary, diagnosis, treatment, etc. Procedural duties will be proportionate to training, skills, and experience. Location: Services shall be performed at the Southern Arizona VA Health care System (SAVAHCS) at 3601 South 6th Ave, Tucson, AZ 85723-0002. Term of Task Order: This task order is effective for a base period of six (6) months from date of award with one (1) six (6) month option period exercised at the discretion of the Government. The Contractor shall perform no service after the task order anniversary date, until the contracting officer authorizes such services in writing. Computerized Patient Record System (CPRS): Urologist Locum Tenen shall enter all patient data (history, evaluation, findings, recommendations, requests for laboratory and x-ray studies, etc.) into the patient's chart electronically. The Urologist Locum Tenen shall have computer and typing skills commensurate with this requirement. Computer training and access will be provided on the first day of service. Sharing of computer access codes is strictly prohibited. The Urologist shall comply with all information security and privacy requirements as dictated by VA and SAVAHCS policy and/or directive. Documentation: Documentation shall be in accordance with the bylaws, policy, and regulations established by the VA and SAVAHCS or the facility in which the care is provided. Consent: The Urologist shall explain procedures, risks, and potential benefits to the patient and/or guardian, obtain proper VA consents, and any other requirements as established by VA and SAVAHCS policy and/or directive. Encounter Forms: A VA encounter form shall be completed on each outpatient treated to include CPT codes and diagnosis. Patients: Patient population requiring services may include inpatient and outpatients with a variety of female and male urologic disorders and diseases. Qualifications: The Urologist shall possess full and unrestricted license and/or certification in a State, Territory, or Commonwealth of the United States or the District of Columbia and a minimum of five (5) years of experience in the field of specialty as outlined in the Scope of Work. Appointment to staff at SAVAHCS is contingent upon successful credentialing and privileging. Appointment to staff is a prerequisite to placement of any clinician performing work under this agreement. Credentialing and privileging shall be completed before providing services. The qualifications of such personnel shall also be subject to review by the SAVAHCS Chief of Staff and approval by the SAVAHCS Director. The Urologist shall be responsible for meeting appropriate JCAHO requirements regarding quality assurance, such as maintaining current credentials and clinical privileges, peer review, participating in criteria development and data analysis and providing guidance in meeting requirements for the Surgical Services. The Urologist Locum Tenen shall be proficient in spoken and written English in accordance with public law 95-201. Contractor's assigned personnel shall be computer literate, have adequate typing skills to perform data entry, and be able to navigate through laboratory related programs. **Seeking information as to availability of any agency providers who would be able to commit to a tour of this length (a base period of 6 months with one (1) six month option exercised at the discretion of the Government) at the Southern Arizona VA Health Care System.
