Appendix G - Work By Contractor Off the Argonne Site Supplemental Conditions Appendix E - Rate Schedule Intellectual Property Provisions, Appendix D-8-C, dated 08/01/2007 Appendix 'C' - Travel Policy for Contractor Personnel - Fixed Price Contracts Argonne National Laboratory Statement of Work, Appendix 'B' dated, 12/19/2015 Argonne Terms and Conditions for Commercial Items Sample Indefinite Quantity Contract Request for Proposal Letter The drilling contractor is tasked to provide the subcontracted work described below on behalf of Argonne National Laboratory in support of the CCC/USDA contract. Depending on the subsurface conditions and the surrounding environment, Argonne will determine the most appropriate and effective type of equipment to deliver the optimal drilling technology: auger drilling rig, full-sized sonic rig, or mini sonic rig. The contractor must be able to provide the requested equipment and operators, and provide the following: Mobilize contractor drill rig as requested, to drill and install monitor wells at project sites at former CCC/USDA grain storage bin sites throughout Kansas, Nebraska, and Missouri. Site selection will be based on determination of priorities by CCC/USDA, and precise well specifications will be provided by Argonne National laboratory. All wells will be completed as either Flush Mount or Stick-up surface installations as required by state regulations and the existing conditions. The Stick-up completions will be required to have three (3) posts cemented in the ground with protective bumper guards around each. All wells are to be developed and completed by the drilling contractor. These wells are to be constructed in compliance with Nebraska Title 178 Regulations for Nebraska installations, or in compliance with Kansas Administrative Regulations 28 30 for Kansas installations, or in compliance with Missouri10 CSR 23-4Monitoring Well Construction Code for Missouri installations. The drilling contractor will complete the State of Nebraska, State of Kansas, or State of Missouri well registration forms, as appropriate, and submit a draft copy for review and comments within 10 days of well completion to: Esther Bowen Argonne National Laboratory EVS 203 9700 S. Cass Ave. Argonne, IL 60439 or another contact as designated by Argonne. The drilling contractor will then make the corrections (if necessary) and register all of the wells as required with the State of Nebraska, State of Kansas, or State of Missouri as appropriate, within 30 days of well completion. Contractor will pay the registration fees directly to the states of Nebraska, Kansas, or Missouri and will provide documentation of payment along with a copy of the completed well registration submitted to the state to: Esther Bowen Argonne National Laboratory EVS 203 9700 S. Cass Ave. Argonne, IL 60439 or another contact as designated by Argonne. Argonne will arrange with its field service contractor to supply all necessary equipment to contain all drill cuttings, mud, development and produced water in plastic lined tanks, barrels, roll-off or trucks at each well location. Argonne will dispose of all produced waste at an appropriate facility.
