SCOPE OF WORK ID#: WS-100-70 THIS IS A SUBCONTRACTING OPPORTUNITY WITH Woodstock Job Corps Center/ADAMS AND ASSOCIATES, INC. and not with the U.S. Department of Labor. INVITATION TO BID VOCATIONAL TRAINING Adams and Associates, Inc., contracting for the U.S. Department of Labor, for the operation of the Woodstock Job Corps Center, a 428 student residential training facility located at 10900 Old Court Road, Woodstock, Maryland 21163. We are currently soliciting written bids for off-site vocational training. The period of performance for this subcontract is February 1, 2015 through January 31, 2016. Written quotes should include cost price per student for the following time period: February 1, 2015 through January 31, 2016. All written bids should be submitted in writing to DeNora Brown, Purchasing Agent no later than January 5, 2015 by COB 5:00 p.m. EST. Please provide a copy of state licenses, professional degrees, certificate of insurance and or certifications as well as a resume. Scope of Services: The work involves the off-site instruction of Nursing Assistant Career Technical Training to students enrolled at the Woodstock Job Corps Center based upon Training Achievement Record (TAR) requirements and contracted performance goals as established by the USDOL, Office of Job Corps. The Center will provide copies of all TAR's and USDOL performance goals to contractor. Instruction is to be provided on a schedule five days per week ( Monday-Friday ), from 9 :00 AM until.3:00 PM, including travel time, excluding Summer and Winter breaks. Detailed requirements are outlined in the following sections. Contractor must: •A. Provide Nursing Assistant class sessions will consist of a maximum of 18 Woodstock students. Classes will include approximately 120 hours of GNA pre-requisite instruction and approximately 156 hours of GNA instruction. A minimum of three (3) sessions will be offered during the Subcontract period. •B. Meet accountability requirement for Outcome Measurement Goals, including graduate career placements, JTMs and the 51% USDOL Minimum Production Rating. 70% Career Technical Trainin g (CTT) Completion Rate 70% Graduate Full-Time Placement Rate 90% Graduate Placement Rate $11.00 per hour wage at initial placement 75% Long-term graduate placement 6-month & 12-month $453 per week long-term graduate placement rate at 6-month follow­ up •C. Maintain Job Corps required student progress, completion and attendance records, including, but not limited to Progress Reports, Attendance and accountability Reports, TAR's and Career Success Rating (CSSR) reports. Attendance records must be completed daily and delivered via fax or email by 9:30 A.M. •D. Document the student's permanent record with the date of entry and date of completion. •E. Will complete the 60 Day Evaluation for students and submit to CTTM or Deputy Center Director, prior to the 60 day deadline date. •F. Ensure that copies of all credentials that are obtained by students are submitted to the Center (CTTM or Deputy Center Director), within 14 days of the beginning of class. Be responsible for the preparation and submission of the following reports to the Deputy Center Director. Weekly Training activity status report Placement activities report Individualized plan for student progress, as needed •G. In addition to the above, instructors shall prepare and submit any reports or information requested by USDOL, the Company, and/or the Center. The Center shall provide training and instruction on report preparation, format and required due dates. •H. Contractor shall provide annual performance evaluations of staff assigned to this subcontract to the Center Director. Center will: •A. To support contractor with th e performance of its duties under this s ubcontract, The Center shall provide: 1. A staff member to serve as liaison between Center and contractor and be responsible for assigning students to the program. 2. Evaluation forms (60 day evaluation) and TARs for the supervisor to complete that document students' progress in the program. 3. Daily transportation for students to and from the work site. 4. Lunch for the students on a daily basis. 5. Oversight and intervention, if a student fails to meet minimum standards of performance or conduct. 6. Health s creenings, to include urine screens and all required inoculations and testing for all participants. 7. Initial completion background checks •B. Staffing requirements, qualifications and procedures: •1. A resume and reference check shall be required for all full-time and Substitute instructors and shall be maintained on file by contractor and the •2. In accordance with the Center's Drug-Free Workplace Policy, all prospective employees of contractor to be assigned to this subcontract shall be tested for drugs and/or alcohol prior to hire. The testing will be arranged through and paid for by the contractor. •3. All prospective employees of contractor to be assigned to this Subcontract shall receive a criminal background check prior to hire. Candidates with prior criminal history may not be employed in any position that has direct interaction with Job Corps students. The background check will be arranged through and by for by the contractor. •4. Contractor will be responsible for providing class coverage for all training days. When an assigned instructor's absence is less than 5 consecutive training days, contractor shall provide a temporary substitute instructor. When an assigned instructor's absence exceeds 5 consecutive training days, contractor shall provide a permanent replacement. Permanent replacement instructors shall meet the requirements for all instructors and shall be in place not more than 30 days from the date the position vacated. Staff vacancies shall not exceed ten (10) days. Adams and Associates, Inc. reserves the right to reject any bids and select the bid deemed most responsible. Bidders should be aware of bid requirements for federally funded projects and act accordingly.
