- Attachment 3 DFARS 252.209-7993 Attachment 2 CLIN Structure Attachment 1 Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subparts 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will NOT be issued. This is a Request for Quote (RFQ) and the solicitation number is M3789815SU00018. The 6th Contracting Squadron (6 CONS), MacDill AFB, Florida contemplates the award of a single contract with Firm-Fixed-Price (FFP) line items for delivery, install, rental and teardown of a modular office trailer. This procurement is being conducted using the Simplified Acquisition Procedures (SAP) authorized in FAR Part 13.5, Test Program for Certain Commercial Items. 1. REQUEST FOR QUOTE The 6th Contracting Squadron (6 CONS), MacDill AFB, Florida, is seeking a Modular Trailer for MARCENT. a. This is a notice that this order is an Economically Disadvantaged Women-Owned Total Set Aside for Small Business (EDWOSB). Only quotes submitted by EDWOSBs verified in the SAM and the SBA's General Login System (GLS) will be accepted by the Government. Any quote that is submitted by a contractor that is not a SB will not be considered for award. b. This requirement is for Economically Disadvantaged Women-Owned Total Set Aside for Small Business (EDWOSB) only. c. All offerors are required to complete and submit Attachment #2 of this solicitation d. All offerors are required to complete and submit Attachment #3 of this solicitation IAW DFARS 252.209- 7993, Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction under any Federal Law. 2. DESCRIPTION OF REQUIREMENT: Reference Attachment #1,Statement of Work (SOW) and Attachment #2, CLIN Structure for a complete description of this requirement. 3. DELIVERY: MARCENT MacDill AFB, FL 33621 4. BASIS FOR AWARD: Award will be made to the lowest priced offeror that provides the items described in the SOW. Any offeror who does not provide specific substantiation of their ability to comply with or perform IAW all requirements of the SOW will not be considered for award. 5. DEADLINE AND DELIVERABLES: Site Visit. A site visit will be conducted on 7 January 2015 at 10:00 AM ET. The site visit is not mandatory to bid on requirement. As prescribed in 5301-9103, insert the following clause: OMBUDSMAN (APR 2014) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution.
