- Please provide an e-mail indicating your interest for this project to Michael.Lane3@va.gov STATEMENT OF WORK: Contractor shall provide Interventional Special Procedure Radiology services to Phoenix VA Health Care System (PHVAHCS) in accordance with the terms and conditions specified herein. The PHVAHCS will provide the Patients, Diagnostic Radiologic Technologists, Radiology equipment and all supplies, necessary for the interventional special procedures. The Contractor shall provide a professional service group which will provide board certified radiologists. On average, the Interventional Special Procedure Radiologists work Monday through Fridays, 0800-1700. Emergency Interventional Procedures are scheduled on an as needed basis for the remaining time. On average, the Interventional Special Procedure Radiologist is called-in for emergencies about 6 times a month. Coverage hours: Seven days per week from 0000-2400 (24 hrs/day). There are two Interventional Special Procedure rooms that could be operating at the same time. The Contractor must have sufficient resources to support the increased workload of the VA population and provide quality care to Veteran patients including adequate qualified/certified personnel, seeing and treating patients in a timely manner and adequacy of planning and treatment. The group must be capable of furnishing physicians who can provide a full spectrum of Interventional Special Procedure services including, but not limited to, aortography, selective catheter placement, venograohy, embolization, percutaneous placement of IVC filter, transcather introduction of intravascular stent(s), and transluminal balloon angioplasty to name a few. The care provided must meet the standards of care established by the American Board of Radiology. All treatments shall be performed at Carl T. Hayden VA Medical Center Radiology Department located at 650 E. Indian School Road, Phoenix, Arizona in accordance with all terms, conditions, provisions, specifications, and schedules herein. The Contractor shall provide Intervention Special Procedures consultation, treatment, interpretation and follow-up care for beneficiaries of the Department of Veterans Affairs as authorized by the Phoenix VA Healthcare System, Phoenix, AZ. DEFINITIONS: Contracting Officer(CO): Acts as the agent of the Government, with authority to negotiate terms, execute legally binding agreements, and administer contracts on behalf of the Government. Contracting Officer's Representative (COR): Typically the requestor or a technical person within the requestor's department who is responsible for the daily technical oversight of the contractor's work. The COR accepts or rejects the contractor's work and reviews/approves contractor's invoices. SPECIFIC CONTRACT REQUIREMENTS CONSULTATION AND TREATMENT: Interventional Special Procedures consultation shall include examination of the patient and the patient's medical record. In performing Interventional Special Procedures, the contractor shall provide complete Radiological or Surgical (Procedural) treatment, in various modalities, as necessary. All Interventional Special Procedure services are to be provided under the direction of a board certified radiologist. The radiological procedures will be identified in the CPT Manual. These codes are known as Radiological Supervision and Interpretation (S&I or RS&I codes), and they define "what you did" (i.e., and arthrogram, a nephrostogram, a bilateral extremity arteriogram). The surgical (procedural) will be identified primarily in the CPT Manual. These codes define "where or how you did it", (i.e., catheter placed in aorta to inject and film abdominal aorta, needle placed in spinal canal to inject contrast for myelography, contrast was injected through t-tube for cholangiogram). The courses of treatment shall further include: Documentation in a patient's chart is critical for demonstrating exactly what was done during a surgical procedure and for supporting the use of particular procedures for documented diagnoses. In the case of an audit, thorough documentation may be the only resource for demonstrating that a particular procedure was performed. Therefore, all E/M, surgical, and S&I services should be clearly delineated as to the exact nature of the services provided. Since most of the procedures performed by interventional radiologists involve multiple surgical codes, it is essential they report documents all of the procedures performed. Contractor employee shall be credentialed to perform all Interventional Special Procedures including, but not limited to Diagnostic arteriography/venography, GU interventions: PCN/stent placement, IVC filter placement, TIPS, Angioplasty/stent placement, Biliary decompression/stenting, Dialysis fistula management: thrombolysis /thrombectomy /angioplasty / stenting, central venous access/management, biopsies and drainages. Contractor employee shall perform direct peripheral Interventional Radiology services in accordance with VA regulations, Joint Commission standards, and Residency Review Guidelines. The contractor employee will also cover other needed sub-specialties as assigned by the Chief of Radiology Service. Contractor shall also be responsible for the following: 1. Provide oversight for provision of services and develop, implement, direct, and monitor operational plans and/or policies to ensure that patient treatment is effective and comprehensive and patient care service is in accordance with community standards of practice. 2. Ensure that consultative services are made available in a timely manner to coordinate patient treatment with medical and surgical subspecialty services. 3. Ensure appropriate professional Interventional Radiology coverage in the inpatient and outpatient clinical services for eligible patients. 4. Ensure that Interventional Radiology is fully prepared to meet Joint Commission and other regulatory agency requirements and standards as well as Medical Center policies and standards. 5. Ensure cost containment for supplies, space, and equipment to promote efficiency and cost effective use of resources. 6. Participate in academic programs at the VA facility. 7. Supervise and train Radiology residents in accordance with standards established by the Residency Review Committee. Residents are assigned to the Medical Center on a rotational basis. 8. Verify/complete all radiology reports within 24 hours of completion of the procedure; this requires use of a computerized Voice recognition system and existing PACS and other Computer systems. 9. Orders will be signed and a post procedure note will be done when the procedure is completed. 10. For first time Radiologists only, a 4 hour training period is provided to become familiar with the computer systems in place. Any additional time needed to learn the systems will not be reimbursed. 11. If the radiologist is unable to verify/complete all reports at the end of a one-week assignment, final payment will be withheld until all reports are verified. If reports are not verified within 4 days after the end of each weekly rotation, then a prorated amount of the fee will be withheld: 10% for each day that the verification process is delinquent over 4 days (calendar days, starting on the Saturday after the week's end), up to a maximum of 60%. SERVICES: The services specified in the Sections entitled Schedule of Supplies/Services and Special Contract Requirements may be changed by written modification to this contract. The modification will be prepared by the VA Contracting Officer and, prior to becoming effective, shall be approved by the VA Under Secretary for Health or his/her designee. Other necessary personnel for the operation of the services contracted for at the VA will be provided by the VA at levels mutually agreed upon which are compatible with the safety of the patient and personnel and with quality medical care programming. The services to be performed by the contractor will be performed in accordance with VA policies and procedures and the regulations of the medical staff by laws of the VA facility. All personnel providing services under this contract shall speak and write English proficiently. Any persons identified as "Key Personnel" (ATTACHMENT A - to be filled out and submitted with the Technical Proposal) to this contract are considered to be essential to the work being performed hereunder. Prior to diverting any of the specified individuals to other programs, the contractor shall notify the Contracting Officer in advance and shall submit justification (including a detailed explanation of the circumstances necessitating the proposed substitution, complete resumes for the proposed substitutes, and any additional information requested by the Contracting Officer) in sufficient detail to permit evaluation of the impact on the program. No diversion shall be made by the Contractor without the written consent of the Contracting Officer, provided that the Contracting Officer may ratify in writing such diversion and such ratification shall constitute the consent of the Contracting Officer required by this clause. Any listing of "Key Personnel" attached to this contract may be amended from time to time during the course of the contract to either add or delete personnel, as appropriate. The Contractor shall be responsible for assuring that their paid employees used under this contract have met the following Occupational Health requirements in accordance with OSHA Bloodborne Pathogens (BBP) Law and the OSHA Tuberculosis Compliance Directive (enforceable under OSHA's General Duty Clause); 1. Baseline Mantoux Tuberculin Skin Test (PPD) (unless the employee has history of positive skin test). This must be offered to the employee free of charge and at a time and place convenient to the employee. 2. Annual PPD, if employee remains employed under this contract for greater than one calendar year. 3. Annual Tuberculosis Education for all employees under this contract. 4. Offered Hepatitis B vaccine, free of charge, if exposure to BBP is likely. Declination Statement must be signed if employee declines Hepatitis B vaccine. 5. In the case of a sharps injury, BBP or TB occupational exposure, the employee must be provided with emergency care. The Contractor shall be responsible for maintaining documentation as required by OSHA and to provide copies to PHVAHCS upon the request of the COTR. QUALIFICATIONS: Personnel assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approval by the VA Facility Director. Each person assigned to work under this contract shall be licensed by American Board of Radiology (ABR) licensing board). WORK HOURS: The services covered by this contract shall be furnished by the contractor as defined herein. The contractor will be required to furnish such services as described below. The following terms have the following meanings: Coverage hours: Seven days per week, as needed from 0000-2400 (24 hours a day). National Holidays: The 10 holidays observed by the Federal Government are: New Years Day Martin Luther King Birthday Presidents Day Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Christmas AND any other day specifically declared by the President of the United States to be a national holiday. Off-Duty hours: The contractor provides 100% on-call as described herein, as arranged with the government. PERSONNEL POLICY: The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for these personnel: - workers compensation - professional liability insurance - health examinations - income tax withholding, and - social security payments. The parties agree that the contractor, its employees, agents and subcontractors shall not be considered VA employees for any purpose. NATIONAL PROVIDER IDENTIFIER (NPI) INFORMATION REQUIRED: All Contractors and health care practitioners who provide billable health care services to the Department of Veterans Affairs, VHA, shall obtain a National Provider Identifier (NPI) as required by the Health Insurance Portability and Accountability Act (HIPPA) National Provider Identifier Final Rule, administered by the Centers for Medicare and Medicaid Services (CMS). This rule establishes assignment of a 10-digit numeric identifier for health care practitioners, intended to replace the many identifiers currently assigned by various health plans. Each practitioner needs only one NPI, valid for all employers and health plans. The NPI may be obtained via a secure website at: https://nppes.cms.hhs.gov/NPPES PATIENT COMPLAINTS: Patient complaints will be managed under the existing procedures established by the VA. If the complaint is medically based, the VA patient's physician will address the issue, otherwise the VA radiology staff will address the complaint. If the complaint is still not resolved, the VA Patient Representative will address the problem to resolution. CONTRACT PERFORMANCE MONITORING: The Contracting Officer's Technical Representative (COTR) is the VA official responsible for verifying contract compliance. After the contract is awarded, any incidents of Contractor noncompliance as evidenced by the monitoring procedures shall be forwarded immediately to the Contracting Officer. CREDENTIALING/PRIVILEGING: a. The contractor will insure that all contract physicians are credentialed and privileged according to the Credentialing and Privileging process as described in VHA Handbook 1100.19 and local policy. No services will be provided by any contract physician prior to obtaining approval by the VA's Professional Standards Board, Clinical Executive Board, and the Director of the PHVAHCS. The contractor will be provided copies of current requirements and updates as they are published. b. Contractor shall allow a minimum of 60 calendar days for review of credentials by the VA Professional Standards Board and the Director or his/her designee. c. Privileges will require renewal at least every two (2) years in accordance with Medical Center and Joint Commission requirements. d. Physicians assigned by the contractor to work at the VA will be required to report specific patient outcome information, such as complications to the Service Line Leader. Quality improvement data provided by the physicians and/or collected by the Service will be used to analyze individual practice patterns. The Service Chief will utilize the data to formulate recommendations for the Professional Standards Board to consider in the renewal of clinical privileges. APPROVAL OF CONTRACTOR PERSONNEL: The VA reserves the right to refuse employment under this contract or require dismissal from contract work of any contractor employee who, by reason of previous unsatisfactory performance at the VA or for any other reasonable reason, is considered by the Contracting Officer to be objectionable. ARRANGEMENT FOR REPLACEMENT STAFF: The contractor shall provide scheduled services throughout the contract period. In the event of the absence of contractor's personnel for any reason, the contractor shall bear the responsibility of providing replacement personnel, who meet the requirements of this contract, to provide the scheduled services. QUALITY MANAGEMENT/ADHERENCE TO POLICIES: The contractor will insure that contract physicians participate in continuous quality improvement programs in the organization as described in VHA Handbook 1100.19. In addition, the services to be performed by the contractor will be done so in accordance with VA policies and procedures, The Joint Commission, federal and state laws, and the regulations of the medical staff by-laws of the VA facility. This includes but is not limited to: outcome measurements, access and levels of care, patient satisfaction, reporting of adverse events, and patient records. Administrative support for VA business will be provided by the VA.
