- Please provide an e-mail indicating your interest for this project to Michael.Lane3@va.gov DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK Services will be furnished to beneficiaries for whom such care is specifically authorized by the Department of Veterans Affairs. These services will be provided to both men and women. There is no requirement to house men and women in different buildings, but if housed in the same building there must be a physical barrier between them. It is understood that the type of patients to be cared for under this contract will normally require care and services above the level of room and board. The contractor shall furnish each veteran authorized care under this contract with the following basic services: 1.Residential room and board. 2.A supervised alcohol and drug free environment which will be staffed on a 24-hour basis 3.Laundry facilities for residents to do their own laundry 4.Therapeutic, Rehabilitative, and Recovery Services determined to be needed by the individual resident in a plan developed by the contractor with consultation by the veteran and the VA case manager and/or other appropriate VA staff. Services which the contractor must be able to furnish include: a.Structured group activities as appropriate - examples include group therapy, social skills training, Alcoholics Anonymous, Narcotics Anonymous, vocational counseling and physical activities as appropriate. b. Collaboration with the VA program staff, which will provide supportive psychosocial services. c.Individual professional counseling, including counseling on self care skills, adaptive coping skills and, as appropriate, vocational rehabilitation counseling, in collaboration with VA program and community resources. d.Assistance to develop responsible living patterns and to achieve a more adaptive level of psychosocial functioning, upgraded social skills, and improved personal relationships. e. Support for an alcohol/drug abuse-free lifestyle. f.Assistance to gain and to apply knowledge of the illness/recovery process. g.Opportunities for immediate learning and/or development of responsible living with a goal of achieving a more adaptive level of psychosocial functioning h.Improving social skill and personal relationships. 5.It is further agreed that beneficiaries receiving care under this contract will receive basic health and personal hygiene maintenance, monitoring of medications, and any other service or supplies normally provided without extra charge. The per diem rate established will be an all-inclusive rate. Absences of the patient from the halfway house in excess of 48 hours will not be reimbursable except with the prior approval of the MAS officer. In the event a beneficiary receiving halfway house care under this contract dies, the halfway house will promptly notify the Department of Veterans Affairs office authorizing admission and immediately assemble, inventory, and safeguard the patient's personal effects. The funds, deposits, and effects left by Department of Veterans Affairs patients upon the premises of the halfway houses shall be delivered by the halfway house to the person or persons entitled thereto under the laws currently governing the patients, unless the beneficiary died without leaving a will, heirs, or next of kin capable of inheriting. When disposition has been made, the itemized inventory with a notation as to the disposition of the funds and effects will be immediately forwarded to the Department of Veterans Affairs office. Should a deceased patient leave no will, heirs, or next of kin, his personal property and funds wherever located vests in and becomes the property of the United States in trust. In these cases the halfway house will forward an inventory of any such property and funds in its possession to the appropriate Department of Veterans Affairs office and will hold them (except articles of clothing necessary for proper burial) under safeguard until instructions are received from the Department of Veterans Affairs concerning disposition. It is agreed that the halfway house will notify the authorizing VA hospital immediately when a medical emergency requiring hospitalization occurs. It is agreed that the veterans will be readmitted to an appropriate VA facility. When such readmission is not feasible because of the nature of the emergency, it is agreed that hospitalization in a non-Federal hospital may he accomplished provided VA authorization is obtained. If hospitalization of a non-emergency nature is required, it is agreed that readmission to the VA hospital will he accomplished promptly. The halfway house agrees and warrants that it does not maintain nor provide dual or segregated patient facilities, which are segregated on the basis of race, creed, color, or national origin. The halfway house may neither require such segregated use by written or oral policies nor tolerate such use by local custom. The term facilities shall include but not be limited to rooms, wards, sections, eating areas, drinking fountains, entrances, etc. It is agreed that the Department of Veterans Affairs will have the right to inspection of the halfway house and all appurtenances by an authorized representative(s) designated by the Department of Veterans Affairs, to determine whether acceptable standards are maintained and that adequate care is being rendered. The halfway house agrees to accept referral of and to provide all services specified in this contract for any person determined eligible by the Chief, Medical Director or his designee, regardless of the race, color, religion, sex, or national origin of the person for whom such services are ordered. In addition the halfway house warrants that subcontracting will not be resorted to as a means of circumventing this position. It is agreed that the VA may readily have access to all medical records concerning the veteran's care in the halfway house. Upon discharge or death of the patient, medical records on all VA beneficiaries will be retained by the halfway house for a period of at least three years following termination of care at VA expense. It is understood that the beneficiary may be provided halfway house care at the expense of the Department of Veterans Affairs for a period not in excess of that stated in the length of treatment contracted, unless an extension of the authorization is provided in writing by the appropriate Department of Veterans Affairs station. It is agreed that the halfway house will submit invoices monthly in arrears for beneficiaries receiving care under this contract. It is understood that payments made by the Department of Veterans Affairs under this contract will constitute the total cost of halfway house care. The halfway house agrees that no additional charges will be billed to the beneficiary or his family, either by the halfway house or any third party furnishing services or supplies required for such care. The halfway house warrants that all applicable fire laws are being complied with and there are no recommendations of fire officials which have not been resolved. It is impossible to determine the exact amount which will be expended under this contract. It is understood that no obligation will be incurred by the Department of Veterans Affairs under this contract until authorizations are issued for halfway house care of specific beneficiaries. The Department of Veterans Affairs agrees to make payment on a timely basis for services rendered in accordance with such authorizations upon receipt of billings submitted by the halfway house at the completion of each month's services. If a beneficiary is admitted to and discharged from the halfway house on the same calendar day, payment will he made for one day. Invoices shall he submitted to the Department of Veterans Affairs facility authorizing admission of the patient. Billing shall include date of admission but not date of discharge.
