Description

This is a SOURCES SOUGHT NOTICE only. Responses to this notice will be used for information and planning purposes. No proposals are being requested or accepted with this notice. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS NOTICE. This notice shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. The Gulf Coast Veterans Health Care System (GCVHCS) has a requirement for Adult Day Health Care (ADHC) services in Mobile, AL. The contractor shall be able to provide all necessary personnel, equipment, and supplies associated with the full range of ADHC Services. Offeror's response to this Notice shall be limited to 10 pages and shall include the following information: "Offeror's company name, DUNS, address, point of contact, title, phone number, and email address "Copy of facility license and liability insurance "A summary of business history, such as how many years providing this type service, and a summary of staff's education and experience. "Offeror's capability to meet this requirement including any current contracts (Civilian or Government). "Offeror's interest in bidding on the solicitation when it is issued "Offeror's type of business (small or large) and socioeconomic status whether Service Disable Veteran Owned, Veteran Owned, Hubzone, 8(a), Woman Owned, Small Disadvantaged, etc The NAICS code is 624120- Services for the Elderly and Persons with Disabilities.) (size standard of $10.0 million). Responses must be received by no later than noon, 12 pm Central, 19 December 2014. Responses may be emailed to Melanie.Grigsby@va.gov using "sources sought number VA256-15-P-0191" in the subject line. Facsimile responses will not be accepted. Responses may be mailed to Melanie Grigsby, Contract Specialist, 400 Veterans Ave (NCO16), Building T102, Room A108, Biloxi, MS 39531-2410. After review of the responses to this sources sought notice, a solicitation announcement may be published on the FBO website. Responses to this sources sought notice are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought notice. All interested parties are reminded to be registered with System Award Management (SAM) at http://www.sam.gov in order to be eligible for award of Government contracts.
