ADDENDA #1 (DECEMBER 16, 2014): ADD attachment. The Request for Quotations (RFQ) document did not attach to the original solicitation release. RFQ #VA255-15-Q-0091 is hereby added as an attachment. ORIGINAL SOLICITATION: ANESTHESIA EQUIPMENT MAINTENANCE & REPAIR SERVICES for the Marion IL VA Medical Center and Evansville IN Health Care Center This is a Request for Quotations (RFQ) - in accordance with FAR part 12 (Acquisition of Commercial Items) and FAR Subpart 13.5 (Test Program for Certain Commercial Items), Network Contracting Office (NCO) 15 is soliciting quotations for furnishing all labor, travel, materials, equipment, tools, supervision, any and all incidentals to provide on-site full service planned maintenance and repair of installed Draeger brand "Apollo" anesthesia delivery systems including endotracheal bronchial aspirators and scavenger units and other accessories. Services are to be provided on-site at both the Marion IL VA Medical Center and Evansville IN Health Care Center. Applicable Statement of Work (SOW), terms and conditions are contained in the attached RFQ #VA255-15-Q-0091document. 1)The procurement will be set-aside for 100% Service Disabled Veteran Owned Small Business (SDVOSB) participation. 2)The NAICS code for this requirement will be NAICS 811219 - Medical and surgical equipment repair and maintenance services, and small business size standard $20.5m. Product Service Code is J065 - Maintenance, Repair, and Rebuilding of Equipment- Medical, Dental, and Veterinary Equipment and Supplies 3)Detailed instructions for submitting a quotation are provided in section E.1 of the solicitation. 4)Quotations will be evaluated on Technical Capabilities, Past Performance and Price in accordance with the evaluation criteria identified in section E.10 of the solicitation. 5)Award will be Best Value. 6)The Government anticipates awarding a one (1) year contract with four (4) one (1) year option periods.
