- This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation number is VA257-15-Q-0339. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-79, effective December 15, 2014. The associated North American Industrial Classification System Code (NAICS) for this procurement is 334516 with a small business standard of 500 employees. All offerors must be registered in the System for Award Management (SAM) prior to award (www.sam.gov). Quotes are being requested and a written solicitation will not be issued. STATEMENT OF WORK 1.1 INTRODUCTION: Contractor shall provide blood reagents that shall be used for blood typing, antigen typing, antibody screening and identification, and cross matching blood as necessary to meet our veterans' transfusion needs. 2.1 SCOPE OF WORK: Supplier will obtain blood bank reagents that will be used to test blood components to meet standards of safety as set forth by the FDA. Supplier will then provide transportation of said blood reagents thru timely shipments to Central Texas Veterans Healthcare System as necessary to support our veterans' transfusion needs. 2.3 CONTRACTOR REQUIREMENTS 2.3.1The Contractor shall provide shall provide all necessary blood bank reagents and testing blood cells required to test patients and our stock of blood units at the Central Texas Veterans Health Care System (CTVHCS), 1901 Veterans Memorial Drive, Temple, Texas 76504. 2.3.2Blood bank reagents shall be delivered prior to the expiration date stated on each reagent and certified that all reagents have been tested in accordance with the required mandates of the Food and Drug Administration (FDA) and the American Association of Blood Banks (AABB). Procedures for obtaining, storage, shipping and delivery must conform to FDA and AABB requirements. By submitting an offer, offeror certifies that their blood bank reagent center is currently licensed with the FDA and accredited by AABB. A copy of license and accreditation shall be submitted with the proposal. 2.3.3All blood bank reagents shall be transported to the CTVHCS in Styrofoam mailing containers packed in ice. 2.3.4Regular deliveries shall be made at scheduled times by the Contractor to CTVHCS to assure that there is always a current in date supply. Reagents will be shipped in accordance with AABB specifications. 2.3.5The Contractor shall have a plan acceptable to CTVHCS for providing blood reagents to meet contract requirements in the event of a VA or other community disaster. The Contractor shall furnish a copy of this plan with its proposal. 2.4TYPES OF BLOOD BANK REAGENT DELIVERIES 2.4.1Regular Deliveries, as scheduled two times per month. 2.4.2Regular Deliveries, as scheduled one time per month. 2.5CTVHCS REQUIREMENTS 2.5.1The Chief, Pathology and Laboratory Medicine Service will ensure an accurate record keeping system is established within the service to provide for efficient management of payment for this contract. 2.5.2CTVHCS will return for credit, within fifteen (15) calendar days of receipt, any reagent lot which is rejected due to breakage or if product is of an unacceptable quality. 2.5.3CTVHCS Blood Bank is under the direction of the Chief, Pathology and Laboratory Medicine Service. After award of a contract, a Contracting Officer's Technical Representative (COTR) will be designated to monitor contractor performance. 2.5.4A Standing Order for the year will be placed with the Contractor. 2.5.5CTVHCS will comply with all FDA mandated recall recommendations. Evaluation of Contractor: The basis for selecting the contractor will be made on three main criteria in the following descending order of importance. 1. Safety. All blood bank reagents must meet or exceed all AABB and FDA standards. 2. Dependability. Manufacturer must be able to meet our blood bank reagent needs consistently 3. Price. The third most important criteria for award of this contract are the lowest possible price. Evaluation of these criteria will be made by the blood bank's Fiscal Administrative Officer, Blood Bank Supervisor, Laboratory's Chief Technologist, and assigned Contract Specialist.
