- List of Provisions and Clauses Applicable to This RFP. Proposed Price/Cost Schedule Performance Work Statement DHS, FEMA Request for Proposal (RFP) No. WX00088Y2015T is being issued to solicit proposals for asbestos testing of air and bulk Samples. All samples to be tested will be provided by the Department of Homeland Security (DHS), Federal Emergency Management Agency (FEMA), Mount Weather Emergency Operations Center (MWEOC), 19844 Blue Ridge Mountain Road, Mt. Weather, VA 20135-2006. Only one award will result from this RFP through the issuance of an all-inclusive, Purchase Order for the Services required in the Performance Work Statement (PWS) which is provided as ATTACHMENT 1 to this RFP. The Period of Performance will include a base period and four consecutive one-year option periods. All Contractors doing business with the federal Government are required to register their DUNS number in the System for Award Management (SAM) website, https://www.sam.gov/portal/public/SAM/ and ensure their record is active. Please provide your company's DUNS number on your proposal. Your pricing proposal should contain costs for the base period and include four option years as indicated in ATTACHMENT 2. A list of provisions and clauses applicable to this RFP is provided as ATTACHMENT 3. EVALUATION FACTORS The proposals received in response to this RFP will be evaluated on a Lowest Priced Technically Acceptable basis. The following criteria must be met in order to be Technically Acceptable: 1. Offeror shall provide evidence that they are properly accredited (IAW the PWS section 2.2, Contractor Qualifications) for testing of Asbestos Samples, i.e., required accreditations include: Asbestos Hazard Emergency Response Act (AHERA) 2. Offeror shall demonstrate ability to provide test sample results within the Government-required response times (five workdays for all but asbestos air samples, which require no more than a 24 hour response time). The subject line of your email should clearly reference RFP #WX00088Y2015T. PROPOSAL SUBMISSION INSTRUCTIONS The Contractor shall submit an electronic proposal via email to Thomas.Crouch@associates.fema.dhs.gov no later than 4:00 PM on Monday, January 12, 2015, or sooner. Total size of email shall not exceed 8MB due to DHS email restrictions. Contractor proposals shall be valid for 30 calendar days after the due date for responses to this RFP.
