The Great Lakes Acquisition Center (GLAC) Southern Tier in North Chicago, IL is soliciting businesses for Optical Dispensing Services at the Jesse Brown VA Medical Center in Chicago, IL and the Adam Benjamin Jr. Community-Based Outpatient Clinic in Crown Point, IN. The contractor shall provide all personnel, equipment, furniture, labor, materials, supervision, and services necessary to effectively, economically, and satisfactorily perform all operations for optical dispensing in accordance with the Statement of Work (SOW) found on RFQ: VA69D-15-Q-0028. The NAICS Code is 446130 with a Size Standard of $20,500,000.00 in revenue. The period of performance shall be for one (1) Base Year of approximately 12 months and four (4) 12-month option years.
