Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 - streamlined procedures for commercial items, in conjunction with FAR Part 13 - Simplified Acquisition Procedures, specifically FAR Subpart 13.5 - Test Program for Certain Commercial Items; and as supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-78, dated November 25, 2014. Solicitation# VA262-15-Q-0187 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm Fixed Price award with a base and two (2) option years. This procurement will be issued under a small business set-aside conducted IAW FAR 13.5; the associated NAICS code is 811219 and small business size standard is $20.5M. See attached SF 1449, Statement Of Work for description and requirements. Place of performance is Veterans Affairs Greater Los Angeles Healthcare System, 11301 Wilshire Blvd., Los Angeles, CA 90073. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. Provisions 52.212-1, Instructions to Offerors - Commercial and 52.212-2, Evaluation - Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on a Pass/Fail evaluation process and award will be made to the offeror who conforms to the solicitation and represents best value to the Government. Submitting Quote: Offeror(s) shall submit their quote on company letterhead and shall include the completed pricing, by individual CLIN, along with name, address, telephone number of the offeror, company's DUNS#, total price per year and a completed copy of Clause 52.212-3 Offerors Representations and Certifications- Commercial Items or vendor's ORCA document. Offerors may submit their pricing on the attached SF 1449. Submit all question(s) to Layton Matthews at Layton.matthews@va.gov no later than December 15, 2014, 2:00 PM Pacific Standard Time (PST). Ensure to reference solicitation # VA262-15-Q-0187 within subject line of the email. Quotes must be received December 18, 2014 by 3:00 PM PST.
