- THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI 53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or 4) Large business firms, to provide for the Edward Hines Jr. VA Hospital, Hines, IL, the lease of a Mobile Computerized Tomography (CT) unit equipped with the GE Lightspeed VCT 64 Slice CT, as described in the summarized Statement of Work (SOW). Summarized Statement of Work: The Contractor shall furnish a Mobile Computerized Tomography (CT) unit equipped with the GE Lightspeed VCT 64 Slice CT. Contractor shall provide materials, tools, equipment, maintenance and repair of all Contractor furnished equipment (including a mobile trailer/unit) necessary for Hines Diagnostic Radiology staff to perform CT procedures utilizing a GE Lightspeed VCT 64 Slice Mobile CT System for the Hines VA Hospital. Contractor shall be responsible to maintain all of the equipment in good working order at all times and shall reimburse the VA for lost time which may result from defective workmanship or equipment failure. All equipment to be used will be Food and Drug Administration (FDA) approved imaging equipment placed at a location designated by the Contracting Officer's Representative (COR) on site at Hines VA Hospital. The entire offering of a Mobilized CT unit service shall meet all local, state, federal, industry, JCAHO, NEC, NFPA, VA, OSHA, and other regulatory standards internal or external that the Hines VA Hospital is currently accountable for. The Contractor shall provide a Mobile CT system that is a full body CT scanner system on site in accordance with the terms and conditions of this contract. The unit shall have the ability to perform a wide range of exams as listed below: The types of exams expected to be performed with the CT: -Complete body imaging such as: CT of the Chest, Abdomen, Pelvis, Extremities. -CT of the Head, Neck and Spine. -CT Angiography (CTA )of the entire body. EQUIPMENT LIST: to be provided at no additional cost to Hines VA Hospital. 1.New hardware and software necessary for the ability to: a.Transmit CT Images to Hines PACS for interpretation. b.Perform required Post processing such as Multi Planar Reconstruction (MPR) and (Maximum Intensity Projections (MIP). c.Power Injector. 2.Dicom network connection to existing AFGA IMPAX version 4.5 3.The mobile unit shall be equipped with an Optical disk archival system. The patient data and optical disk is the property of the Hines VA Hospital. 4.The mobile unit shall be equipped with CD-R burning capabilities as a backup to network transfer of data. ACCESSORY EQUIPMENT: Accessory equipment that shall be provided by the Contractor: 1.Handicap accessibility. 2.Intercom system between the control room and scanning room EQUIPMENT SPECIFICATIONS: The following minimum performance characteristics of the equipment are required: 1.Contractor will be required to make whatever adaptations are necessary so that the Contractor's equipment is compatible with the VA furnished services defined herein. 2.The CT shall provide images with consistently high technical quality. 3.The CT Unit shall have a multi-slice detector and be capable of acquiring a volume of images within one breath hold for different body parts. 4.The CT unit shall be capable of acquiring data for anatomic localization. 5.The CT system shall have attenuation correction algorithm(s) that are designed to use non-contrast enhanced CT data. 6.The CT review station shall have software packages that are designed to reconstruct /post-process MPRs and MIPs. 7.The review stations shall have software tools that are capable of providing three orthogonal planes of the CT images alone. 8.The Mobile CT unit shall be ready for use regardless of outside environmental conditions. 9.The Mobile CT unit shall maintain a temperature to assure proper operation of the scanner without frequent calibration and provide for patient comfort. 10.Meet all federal, state and local fire and safety requirements. 11.Telecommunications: The mobile unit shall be equipped with an outlet connection capable of transmitting data. 12.The Government will provide the connection required for the transmission of images and data. The Contractor's mobile unit must be able to connect to VISTA Imaging Network and to transmit all images to VISTA Imaging to include being compatible to Dicom 3 imaging standards. (PACS interface contact person shall be provided to the successful Contractor during the post-award orientation). Additional Requirements/Mobile Trailer: Contractor shall provide a trailer that in addition to housing the CT unit and associated equipment meets the following physical requirements: "Has been approved by the FDA and meets or exceeds minimal standards for the type of equipment being provided as defined herein. "This Mobile CT unit shall interface to an enclosed access ramp so as to provide both security of VA and Contractor property and to provide a barrier to outside weather conditions. "The Contractor's CT trailer shall provide the space sufficient for conducting CPR and cardiac code if needed. This requirement will be awarded on a lowest-price technically acceptable basis. Tentative period of performance is for a six (6) month base period, plus two 3-month option periods, to be exercised at the Governments discretion. All interested firms who can meet the requirements stated above should respond, in writing, with: 1.Company name. 2.Address. 3.Point of contact (name/telephone number/e-mail address). 4.DUNS number. 5.Tax ID number. 6.Indication of which business category (SDVOSB, VOSB, Small Business or Large Business) for which the organization qualifies. 7.A capability statement that addresses the organizations qualifications and ability to perform as a contractor for the requirement depicted in the SOW. Submit responses to the Contract Specialist, Thomas Chesak, at thomas.chesak@va.gov no later than 11:00 a.m. (Central Standard Time), 12 December 2014. Mobile CT Unit Rental should be referenced on all correspondence regarding this announcement. TELEPHONIC INQUIRIES WILL NOT BE ACCEPTED. The NAICS Code 621512, Diagnostic Imaging Centers, is applicable to this acquisition; the size standard is $14.0 M. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract. The Government will use responses to this notice to make an appropriate acquisition decision. This is the only notice, no other copies of this notice will be provided.
