- THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and shall not obligate the Government to award a contract. The purpose of this Sources Sought Announcement is for market research to gain knowledge of potential qualified sources interested and capable of performing the work and obtain their size classification relative to the North American Classification System (NACIS) Code 333415 with a size standard of 750 employees. A.GENERAL INFORMATION Background: The Hines Illinois VA healthcare System intends to purchase an Ultra Low Upright Freezer for use in its Laboratory. Scope of Work: The contractor shall provide the following as listed below in the SOW. This contract is for the purchase of a Thermo Scientific REVCO -86C Ultra Low Upright Freezer w/ built in 6" Ink Chart Recorder, or similar piece of equipment which meets the salient features listed below. This is a BRAND NAME OR EQUAL requirement which will be used by the VA Medical Center. This is a salient requirement, and is not flexible. The following are the minimum salient characteristics that must be met: Please note that this is a salient requirement, and is not flexible. The following are the minimum salient characteristics that must be met: The contractor will supply an ultra-low freezer with a built in chart recorder with the following specifications: "Temperature range must begin at -50oC and must reach -86oC. "Unit must come equipped with a built-in chart recorder for continuous temperature monitoring. "An ergonomic lockable door handle must be present for storage in a multi-user environment. "17.3 cubic feet capacity and overall dimensions of 33.3"W x 77.9"H x 38.9"D are desirable due to laboratory space limitations. "Rust and corrosion proof, stainless steel interior. "Five interior compartments to reduce cold air loss. "Visual and Audible Alarms Included for: "Temperature deviation from your high and low set points "Power failure "Door ajar "Hot condenser "Probe failure "Low battery on the control system Acquisition of Thermo Scientific -86C Ultra Low Upright Freezer (17.3 cu ft.), or equivalent freezer meeting these specifications must include the following. This task includes purchase and delivery of the freezer to Hines VA Hospital campus. The warehouse at the Hines VA Hospital will receive this product and deliver to the end user, where the user will move the item into position, complete the installation according to the manufacturer's electrical requirements, and begin using the machine. REQUESTED INFORMATION: STATEMENT OF CAPABILITY: Submit a brief description (five [5] pages or less) that shows your company can provide the required services. One additional page may be included explaining experience in performing these services to the VA, other government agencies, or for private facilities. Potential sources should identify business name, point of contact, business size and any applicable business socio-economic category (i.e. small business, SDVOSB, VOSB, SB, etc), DUNS number, address, and any additional information felt to be pertinent. All vendors must be registered in SAM https://www.sam.gov/portal/public/SAM/ prior to contract award. Responses to this notice should be electronically mailed in PDF format or word format to Cheryl.Homison@va.gov. Telephone responses will not be accepted. Written responses must be received no later than 2:00 PM (EST), Wednesday, December 10, 2014.
