Description

SYNOPSIS: This is a request for information (RFI) in accordance with FAR 15.201(e). This is not a solicitation for quotes, neither unsolicited proposals nor any other kinds of offers will be considered in response to this RFI and there will be no contract award from this RFI. The purpose of this Request for Information is to obtain the latest information in seeking potential businesses in an effort to assist the Government in better understanding the types of concentrates for hemodialysis solution available in the market place and the terms and conditions under which they are sold. Your company's response to this request for information will assist the Government in developing its requirements for a possible mandatory national contract. The Department of Veteran Affairs Strategic Acquisition Center (SAC) is interested in procuring hemodialysis supplies. The hemodialysis supplies is to be provided to VA Medical Centers throughout the Unites States that treats patients using hemodialysis and peritoneal dialysis process, during a five (5) year specified period (one base year, plus four option years). Interested parties shall provide a written response that your company can meet the following requirements: 1)Identify all dialysate proportioning ratios offered by your company. 2)List all modes of supply (i.e. drums, cases, gallons) for Bicarbonate and Acid Concentrate offered by your company. 3)Company shall be registered with the Food and Drug Administration (FDA). 4)Products shall be FDA approved. Title 21 Food and Drug, Chapter I- Food and Drug Administration Department of Health and Human Services Subchapter H- Medical Devices, Part 876 Gastroenterology-Urology Devices, Subpart F-Therapeutic Devices, Section 876.5820 Hemodialysis systems and accessories Interested parties shall provide a written response to each of the following questions: 1)What types of bicarbonate and acid concentrate do you distribute and how is it packaged? Please provide a product listing. 2)Does your company provide delivery of items to the dialysis unit? 3)Does your company provide pumping of acid concentrate from the drum into our concentrate holding tanks? 4)What is your company's ISO 9000 certification status? 5)Does your company's products comply with the current ANSI/AAMI/ISO Standards 13958:2009 and 11663:2009? 6)What are your company's procedures in the event of a product shortage? (Please provide detailed information) 7)Can your company's products be used on hemodialysis equipment from various manufacturers? Yes/No If yes, please provide information on machines compatible with your products. Responses to this RFI may include general information regarding your company name, point of contact (having authority and knowledge to discuss responses with Government representatives), address, phone number, email address, type of business, such as Large, Small Business, 8(a), HUBZONE, WOSB, Veteran Owned, Small Disadvantaged Business, Service Disabled Veteran Owned, etc.), in accordance with the Small Business Administration. Please note, all SDVOSB and VOSB must be verified by the office of Small Disadvantaged Business Utilization Center for Veterans Enterprise (CVE) prior to receiving any award. The North American Industry Classification System (NAICS) code is 339113 with a business size standard of 500 employees. Please identify your company's NAICS code. Please identify your company as a manufacturer or distributor. Please provide information if your company offers these products on GSA/FSS Schedule (i.e. contract number). The Government requests that no proprietary or confidential business data be submitted in a response to this notice. However, responses that indicate the information therein is proprietary will be properly safeguarded for Government use only. RESPONSE INFORMATION: All responses to this RFI must be submitted electronically (via email) to Antionette Collins-Somerville, Contract Specialist, at Antionette.Collins-Somerville@va.gov in either Word or Adobe Portable Document Format (PDF) on or before December 12, 2014, 2014 at 12:00 p.m. EST, in order to be considered. Do not contact medical center staff in regards to this requirement, as they are not authorized to discuss matters related to this procurement action. All questions should be submitted in writing to the Contract Specialist, Antionette Collins-Somerville, Contract Specialist, at Antionette.Collins-Somerville@va.gov THIS NOTICE IS NOT A REQUEST FOR PROPOSALS. This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. Confidentiality, No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).
