Description
This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR Subpart 12.6, using Simplified Acquisition Procedures under the test program for commercial items found at FAR 13, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This combined synopsis/solicitation SHALL be posted on both FEDBIZOPPS and NECO (https://www.neco.navy.mil/). The RFQ number is N00604-15-T-3010. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-77 and DFARS Publication Notice 20141113. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The NAICS code is 611430 and the Small Business Standard is $11M. The proposed contract is 100% set aside for small business concerns in accordance with FAR 19.5. The Fleet Logistics Center Pearl Harbor Regional Contracting Department requests responses from qualified sources capable of providing: CLIN0001: RISK REDUCTION, RESILIENCE AND SUICIDE PREVENTION TRAINING. SERVICES IN ACCORDANCE WITH THE ENCLOSED PERFORMANCE WORK STATEMENT. INCLUDES ALL FEES FOR CONSULTATION, DEVELOPMENT, PREPARATION, DELIVERY AND TRAVEL. QTY: 25 SESSIONS Period of performance is 26 January 2015 thru 18 August 2015; please see attached PWS for delivery locations. All responsible sources may submit a quote which shall be considered by the agency. Procedures in FAR 13.106 are applicable to this procurement. While price will be a significant factor in the evaluation of offers, the final contract award will be based on a combination of factors - a combination of price and technically acceptable (LPTA). The Contracting Officer will rank offerors from lowest to highest price, review the technical proposal of the lowest price offeror, and, if that offer is technically acceptable, make the award without further evaluating the technical proposals of the other offerors. To be technically acceptable, the offeror shall at a minimum address the following: a. Describe its technical approach to accomplish the requirements of the PWS in sufficient detail to establish technical acceptability. b. Recognition and or credentials by one or more of the following Suicide Prevention Organizations: American Foundation of Suicide Prevention (AFSP), The American Association of Suicidology, The Jed Foundation, National Alliance on Mental Illness (NAMI), The National Association of State Mental Health Program Directors, Man Therapy, Mental Health America, National Organization for People of Color Against Suicide (NOPCAS), Substance Abuse and Mental Health Services Administration (SAMHSA), SAMHSAs National Mental Health Information Center, SAVE - Suicide Awareness Voices of Education, Suicide Prevention Action Network USA, Suicide Prevention Resource Center, The Trevor Project, or the Yellow Ribbon Suicide Prevention Program. c. Knowledge and familiarity with the DoD, specific CNO goals and strategic objectives for the Navy-wide Suicide Prevention program. d. Experience and ability to orally communicate complex resilience, risk reduction and suicide prevention issued to diverse audiences and demographics from Senior Flag officers to junior enlisted personnel. Responses shall not be a restatement of the requirements but shall be comprehensive, well-conceived and include detailed approaches to accomplishing the tasks presented in the PWS. Contractors simply providing general statements or paraphrasing/parroting the PWS in whole or in any part may result in a rating of unacceptable. System for Award Management (SAM).
